| Objective:To observe the clinical efficacy of electronic needle twister instrument in treating limb dyskinesia after ischemic stroke and its effect on serum BDNF level.Methods:Sixty patients with limb dyskinesia after ischemic stroke were randomly divided into a treatment group and a control group.Two groups were 30 cases respectively.Both groups were given conventional drug,rehabilitation and acupuncture treatment.In the treatment group,Baihui(GV20),the upper 1/5segment as well as the middle 2/5 segment of the anterior parietal temporal oblique line on the contralateral side of the affected limb were selected as scalp acupuncture points.After the arrival of Qi,electronic twister instrument was used for twisting treatment,the frequency was 200 revolutions/min,and the duration was 3 minutes.Qinglenengyuan(TE11),Bingfeng(SI12),Baxie(Extra 28),Waiguan(SJ5),Ququan(Liv8)and Zusanli(ST36)on the affectd side were alternatively selected with appropriate stimulation according to individual clinical conditions.Retained the needle for 30 minutes.The control group chose the same points as the treatment group,and did not use the electronic twisting device,and the needle was retained for 30 minutes.Patients in the two groups were treated with acupuncture once a day for 14 days.Recorded and observed the National Institutes of Health Stroke Scale(NIHSS),abbreviated Fugl-Meyer score,serum BDNF levels and the overall efficacy of the two groups before and after treatment.Result:1.There was no significant difference in gender,age,Brunnstrom score,NIHSS scores,Fugl-Meyer scores and the serum BDNF content between the two groups before the treatment(P>0.05).2.On one hand,intra-group comparison indicated that the NIHSS scores were significantly lower than before treatment;while the Fugl-Meyer scores as well as the serum BDNF content were significantly higher than before treatment;in addition,the differences above were extremely significant(P<0.01).On the other hand,comparison between two groups illusrated that the NIHSS score of the treatment group was lower than that of the control group,while the Fugl-Meyer score and the serum BDNF content of the treatment group were significantly higher than the control group,furthermore,the differences above were significant(P<0.05).3.The results of the overall effective rate after treatment of the two groups showed that the total effective rate of the treatment group was 86.67%,and the total effective rate of the control group was 76.67%.The difference was statistically significant(P<0.05).Conclusion:1.Both a cupuncture alone and electronic needle twister instrument have a certain effect on the treatment of limb dyskinesia after ischemic stroke,and the overall effect of electronic needle twister instrument is more significant.2.In terms of improving neurological deficits and motor dysfunction,both methods have a certain effect,and the effect of electronic needle twister instrument is more significant.3.In terms of promoting the expression of BDNF in patients with ischemic stroke,both methods can increase the content of endogenous BDNF in the serum to a certain extent,and the effect of the electronic twisting needle instrument is more significant,thus it is inferred that this therapy is more helpful to repair damaged nerves. |
