| Objective: Through the analysis of the amount of health resources in Chongqing and the input-output situation,understand the basic situation of the city’s health resource allocation,and conduct research and analysis from the three aspects of the fairness,efficiency and resident satisfaction of resource allocation to clarify Chongqing The current situation of the allocation of health resources in the city,summarizes the trend of health resource allocation,finds out the problems in the allocation of resources,and puts forward opinions and suggestions for optimizing the overall allocation level of health resources,provides a reference for the regional health planning of Chongqing,and provides a theoretical basis for the construction of a "healthy Chongqing".Method: Use literature research method to summarize the domestic and foreign health resource allocation research,summarize evaluation indicators and analysis methods,use Lorentz curve,Gini coefficient and Theil index to evaluate Chongqing’s health from the three dimensions of population,geography and economy Resource allocation fairness;use data envelopment analysis to compare the efficiency of health resource allocation in Chongqing from horizontal and vertical;use questionnaire survey and descriptive statistical methods to analyze Chongqing residents’ health resource utilization satisfaction and its influencing factors.Results: 1.The basic situation of the allocation of health resources in Chongqing: In 2018,Chongqing had 6.47 beds per 1,000 population,6.15 health technicians,2.24 physicians,and 2.79nurses;Chongqing government health expenditure was 20.75 hospitals and 1688 health institutions There are 64,676 beds,67,724 health and technical personnel;49,922,500 medical visits,and 1.435 million discharged.From 2014 to2018,the amount of health resources per capita and input and output have shown an upward trend year by year.2.Equity of allocation of health resources in Chongqing: In 2018,the Lorentz curve of health resources allocated by economy in Chongqing has the smallest curvature,and the curve has the largest curvature according to geographic allocation;the Gini coefficients of health resources allocated by economy and population are both less than 0.4,The range according to geographic configuration is 0.1730 to 0.6093;the total Theil index of various health resources is between 0.0146 and 0.2887,and the contribution rate between groups in the dimensions of population and geography is greater than the contribution rate within groups.3.The efficiency of health resource allocation in Chongqing: In 2018,25 of the 38 districts(counties)of Chongqing City had invalid DEA of health resource allocation efficiency,and some areas had insufficient output and redundant input;2009-2010,2014-2016 health Resource allocation efficiency DEA is invalid,and returns to scale are diminishing after 2013.4.Satisfaction with Chongqing’s health resources: Most residents(70%)are more inclined to choose secondary and tertiary medical institutions for medical treatment after falling ill.The reasons include sufficient human resources(25.6%)and excellent service level medical hardware equipment resources(23.5)%)Wait.Among the various health resource allocation satisfaction levels,practicing(registered)physicians scored the lowest(2.16±0.962),and primary medical and health institutions scored the highest(2.67±0.987);the results of multiple linear regression showed that age and medical insurance type are the overall satisfaction of health resources Independent influencing factors(p<0.05).Conclusion: The total amount of health resources in Chongqing has increased in the past five years,but the fairness of some resources allocation has declined;the fairness of resources allocated by economy and population is better than allocation by geography;the allocation of health resources varies greatly among regions;and the allocation of resources The structure still needs to be optimized.The current health resource allocation efficiency problem reflects the misallocation of resource supply and demand.The main reasons for the low resource allocation efficiency include unreasonable resource allocation and insufficient utilization of existing resources.Residents in Chongqing are not satisfied with the utilization of health resources.The utilization of health resources is significantly affected by residents’ subjective tendency to go to a doctor.However,the current residents’ concept of health resource utilization has not changed,and high-quality health resources have not been effectively sinking.Based on the research results,the following suggestions are put forward: play the role of policy guidance,improve the health resource allocation system;attach importance to the equity of health resource allocation based on geographic allocation,and narrow the gap in resource allocation between regions;improve the structure of health resource allocation and pay attention to the allocation of health human resources;strengthen health Flexible allocation of resources,pay attention to the utilization of stock health resources;improve the service level of primary medical and health institutions,and guide residents to change the concept of health resource utilization. |
