| Objective:AMH level through this study collects and infertility patients,TCM syndrome types,sex hormone levels and related parameters of the receptivity of endometrium and endometrial pathologic results and other data,study of infertility patients AMH level and TCM syndrome type and correlation between endometrial condition such as the physical and chemical indexes,and explore its influence on the receptivity of endometrium,To provide certain data support for clinical use of AMH level to determine the endometrial receptivity of infertility patients,and to explore the distribution characteristics of TCM syndromes of infertility patients with different AMH levels.Method:By collecting infertility patients who visited the Reproductive Center of the First Affiliated Hospital of Tianjin University of Traditional Chinese Medicine from February 2019 to February2021,the data of age,BMI,course of disease,TCM clinical information and laboratory tests of the patients were collected.Patients were divided into low AMH group,normal AMH group and high AMH group according to different AMH levels.The differences in age,course of disease,TCM syndrome type,sex hormone level,endometrial lesions and other aspects of patients in different AMH levels were compared to explore whether the differences affected endometrial receptivity.Factors that may be correlated with AMH level were screened out,and Logistic regression model was established to analyze the correlation of the screened factors.A total of 281 valid research cases were collected in this study,and the data were input using Excel software and imported into SPSS21.0 software for statistical analysis.Results:In this study,a total of 281 effective cases were obtained according to the Na/E standard,including 93 patients in the low-level AMH group and 94 patients in the normal and high-level AMH groups.(1)In this study,infertility patients with different AMH levels showed no statistical differences in age and course of infertility(P>0.05).(2)In this study,infertility patients with different AMH levels had significant differences in BMI level and ovulation regularity(P<0.05).Further study found that high AMH level was positively correlated with overweight and obesity(P<0.05).(3)In this study,there were differences in TCM syndromes of infertility patients with different AMH levels(P<0.05),in which deficiency syndrome was more than evidence in the low-level AMH group,and deficiency syndrome was more than evidence in the high-level AMH group.Low AMH level was positively correlated with syndrome of Shenyangxu,Ganyuqizhi,and Yuzhibaogong(P<0.05),while high AMH level was positively correlated with Ganyuqizhi,and Tanshizuzhi(P<0.05).(4)In this study,there were no statistical differences in PRL and E2 among infertile patients with different AMH levels(P>0.05),but there were differences in FSH,LH and T(P<0.05).Further study found that low AMH level was positively correlated with FSH(P<0.05),and negatively correlated with LH(P<0.05).High AMH level was negatively correlated with FSH(P<0.05)and positively correlated with LH(P<0.05).(5)In this study,there was no statistical difference in endometrial blood flow state and endometrial morphology among infertile patients with different AMH levels(P>0.05),but there were statistical differences in endometrial thickness and endometrial pathology(P<0.05).The endometrial receptivity of normal AMH group was better than that of high and low AMH group.High AMH level was positively correlated with endometrial thickness,and negatively correlated with endometrial hyperplasia without atypical hyperplasia(P<0.05).Conclusions:(1)There were significant differences in BMI among infertile patients with different AMH levels,and high AMH level was positively correlated with overweight and obesity.(2)different AMH level of infertility patients with obvious differences in the aspect of TCM syndrome types,and low AMH level with Shenyangkuixu,Ganyuqizhi,Yuzhibaogong,higher AMH level and Ganyuqizhi,Tanshizuzhi were positively correlated,but for infertility in combination with the physical and chemical indicators to provide certain reference basis for clinical evidence-based medicine.(3)The levels of FSH and LH in infertile patients with different AMH levels were significantly different,and low AMH level was positively correlated with FSH and negatively correlated with LH;High AMH level was negatively correlated with FSH and positively correlated with LH.(4)AMH level may affect many factors such as endometrial thickness,blood flow,morphology,pathology,etc.In particular,high AMH level is correlated with endometrial thickness and endometrial hyperplasia without atypical hyperplasia.AMH can affect endometrial receptivity by affecting the above factors,so the endometrial receptivity in the normal AMH group was better than that in the high AMH and low AMH groups. |
