The Clinical Observation On Acupoint Catgut Embedding Treating Polycystic Ovary Syndrome Of Phlegm Dampness Type

ObjectiveTo observe the clinical efficacy of acupoint catgut embedding therapy in the treatment of polycystic ovary syndrome(PCOS),and compare it with Diane-35,so as to provide the basis for the clinical treatment of PCOS.Methods60 patients with PCOS of phlegm dampness type were randomly divided into acupoint catgut embedding group and Western medicine group,30 cases in each group.Both groups were treated with diet adjustment,exercise therapy and oral Chinese medicine Cangfu daotan decoction.On this basis,the catgut embedding group selected Zhongwan(RN12),Guanyuan(RN4)and bilateral Tianshu(ST25),Daheng(SP15),Shuidao(ST28),Guilai(ST29),Zigong,Zusanli(ST36),Fenglong(ST40)for catgut embedding therapy.Avoiding menstrual period as far as possible,and catgut embedding therapy was usually carried out on the 5th,15 th and 25 th day after menstruation,three times as a course of treatment,three consecutive courses of treatment.If menstruation is delayed for more than 7 days,the first catgut embedding of the second course of treatment will be started on the 35 th day,and so on.If menstruation ahead of 7 days,then the third time of the last course of treatment without catgut embedding,in order to avoid the menstrual period.The third course of acupoint catgut embedding time nodes refer to the second course.The western medicine group began to take Diane-35 orally on the 5th day of menstruation or withdrawal bleeding,and took one tablet before going to bed every day for 21 consecutive days as a course of treatment.Start the second course of medication from the 5th day of menstruation,and start the next course of medication from the 35 th day of the first menstruation if there is no menstruation for more than 7 days.The treatment lasted for 3 courses.The therapeutic effect was evaluated after 3 courses.Menstrual cycle,TCM syndrome score,obesity index,sex hormone level and glucose metabolism level were used as observation indexes to observe the changes of each index before and after treatment in the two groups.All statistical calculations were analyzed by spss26.0 software,P>0.05 was no statistical significance,P<0.05 was statistically significant,P<0.01 was significant difference.Results1.Before treatment,there was no significant difference in age,body mass index(BMI),waist circumference,luteinizing hormone(LH),follicle stimulating hormone(FSH),LH/FSH,testosterone(T),fasting blood glucose(FPG),fasting insulin(FINS),insulin resistance index(HOMA IR),TCM syndrome score and menstrual cycle(P>0.05),suggesting that the two groups had comparability.2.Obesity index: The BMI and waist circumference of the two groups decreased significantly(P<0.01)after treatment than before treatment.The difference between the two groups was significant(P<0.01),suggesting that the effect of reducing BMI and waist circumference in the embedding group was better.3.Sex hormone level: Compared with the group before and after treatment,the LH,LH/FSH and T of the two groups were decreased(P<0.05),FSH had no significant change(P>0.05).After treatment,there was no significant difference between the two groups(P>0.05),suggesting that the two intervention methods were equivalent to the reduction of LH,LH/FSH and T.4.Glucose metabolism index: There was no significant change in FPG than before treatment(P>0.05),and the FINS and HOMA-IR decreased after treatment(P<0.01),indicating that both intervention methods could reduce fasting insulin and reduce insulin resistance.After treatment,there was no significant difference of the decrease of FPG between the two groups(P>0.05).The difference of FINS and HOMA-IR between the two groups was statistically significant(P<0.05),suggesting that catgut embedding group has better effect in reducing insulin resistance.5.Menstrual cycle and TCM syndrome score: Before and after treatment,the scores of menstrual cycle and TCM symptoms of the two groups were improved,and the difference was statistically significant(P<0.01),suggesting that the two groups can significantly improve the menstrual cycle and TCM symptoms.After treatment,there was no statistically significant difference between the two groups in the menstrual cycle(P>0.05),suggesting that the two intervention methods have the same effect on the recovery of menstrual cycle.There was significant difference of the decrease of TCM syndrome score(P<0.01),suggesting that catgut embedding group is better than western medicine group in improving TCM syndrome.6.There was no significant difference in the comprehensive effects between the two groups(P>0.05),suggesting that the comprehensive effects of the two groups was similar.In the follow-up menstrual cycle group,the follow-up effect of catgut embedding group was the same as that after treatment(P>0.05),and the follow-up effect of Western medicine group decreased(P<0.05).The difference of the follow-up menstrual cycle between groups was statistically significant(P<0.05),suggesting that the long-term effect of catgut embedding group in adjusting menstrual cycle is better.Conclusion1.Both acupoint catgut embedding group and Western medicine group can reduce BMI,waist circumference,improve sex hormone level,menstrual cycle and TCM syndrome score,reduce insulin resistance.2.The two groups had the similar effect in improving the comprehensive curative effect,menstrual cycle and sex hormone level;the catgut embedding group was better than the western medicine group in reducing BMI,waist circumference,insulin resistance and TCM syndrome score.3.In the long-term effect of adjusting menstrual cycle,acupoint catgut embedding group had better effect.
