Objectives:1.Retrieve and evaluate the relevant evidence of home fall risk intervention for stroke patients at home and abroad,and integrate and summarize the evidence to form the best evidence of home fall risk intervention for stroke patients,and provide guidance for clinical nurses on the intervention of stroke patients at home Evidence basis.2.Transform the best evidence on the risk of falls at home for post-stroke patients into clinical evidence,compare the gap between clinical practice and the best evidence,analyze obstacles,reduce the incidence of falls at home for post-stroke patients,and increase the enthusiasm of nursing staff to apply evidence-based practice evidence.Methods:1.Systematic retrieval of relevant domestic and foreign databases on the evidence of stroke patients falling at home.The types of documents include guidelines,systematic reviews,evidence summaries,expert consensus,and norms.The search time limit is from the establishment of the database to September 31,2020.Two trained researchers conduct quality evaluation and evidence integration,and summarize and integrate the evidence in the literature that meets the quality requirements.2.The content analysis method of descriptive research was used to explore the status quo of the pilot departments for the management of falls at home for post-stroke patients and their attitudes towards evidence-based evidence transformation.Through interviews with nurses in the neurology department of a tertiary A hospital in Shanxi Province,and combined with the results of qualitative analysis,7 review standards were formulated to implement evidence-based interventions for the prevention and care of home falls for post-stroke patients in pilot departments.The review criteria determine the gap between clinical practice and the best evidence,and analyze barriers..Results:1.A total of 13 articles were included in the evidence summary of this study,including5 guidelines,1 best practice recommendation,5 systematic reviews,1 guideline,and 1evidence-based opinion.Finally,a total of 16 pieces of evidence including assessment,health education,daily activities,and family environment were formed.2.Through the project review,this study clarified that there is a gap between the clinical practice and the best evidence for nursing interventions for the risk of falling at home for patients after stroke.The review team determines the implementation of each review standard.2 review standards are excellent(over 75%),5 review standards are medium(50%-75%),and 0 is low(below 50%).Based on the implementation situation,the review team determined that nurses lacked awareness of the risk of falls for discharged patients after stroke;community hospitals were not fully functional and patients could not receive continuous nursing services;formal health education and nursing interventions increased the workload of nurses;patients And their family caregivers are not clear about the impact of the family environment on falls,and the four obstacles of inaccurate health education and education,and propose solutions.Conclusion:1.This study is based on evidence-based nursing practice and integrates operable interventions for the risk of falling at home for post-stroke patients,guiding clinical nurses to accurately identify high-risk fall patients,and providing evidence-based basis for the correct and scientific management of fall events in post-stroke patients.2.The process of qualitative interviews and analysis of obstacles and suggestions for countermeasures provides scientific basis for clinical nurses’ practice,promotes the transformation of evidence-based evidence in clinical evidence,effectively improves the quality of care,and reduces the incidence of falls at home for stroke patients.
