Objectives:1.To investigate the needs of home care of patients with stroke and their families,and to analyze the influencing factors of home nursing needs of stroke patients and their families.2.Based on the investigation of the current situation,a home nursing intervention program was designed to meet the needs of stroke patients and their families.3.To explore the program in young stroke patients and their families in the application effect.Methods:1.The investigation: Using convenience sampling method in 3-5 of 2016 to investigate the status of 205 stroke patients and their families in a department of neurology of a top three hospital in Shanxi province.The self-made stroke patients and their family nursing needs questionnaire were used as investigative tools to understand the home care needs of stroke patients and family members of the status quo and its impact factors.2.Intervention study: A sample of 110 middle-aged and young stroke patients and their families in Shanxi Province was selected by convenient sampling method in June2016 for a six-month home care intervention.Fifty-five intervention groups and control group were divided into intervention group and control group according to the random number table.Nursing care and follow-up on the basis of time-based theory to take home care;control group received routine neurological nursing and follow-up,respectively,discharge,discharge 3 months and 6 months to evaluate the intervention effect.Results:1.Survey results(1)The total home nursing needs of stroke patients was 3.25 ± 2.89.Univariate analysis of variance(ANOVA)showed that age,gender,marital status,educational level,working status,type of stroke,number of strokes,sequelae,post-discharge therapy and activities of daily living were the influencing factors.The results showed that the ability of daily living,the sequelae and educational level were the main influencing factors(F =67.02,P<0.001).(2)The total score of family nursing needs of stroke patients was 3.24±0.35.Univariate analysis of variance(ANOVA)showed that sex,marital status,patient relationship,and cohabitation were the factors influencing factors.Single factor ANOVA was introduced into multiple linear regression analysis.The results showed that gender,marital status,(F =17.59,P<0.001).2.Intervention results(1)The self-care ability and self-efficacy of the two groups before intervention were analyzed by two independent samples t test,the results showed: the difference was not statistically significant(P>0.05).After intervention,the self-care ability of the intervention group was higher than that of the control group(P<0.05).The results of the repeated analysis showed that the difference was statistically significant(P<0.001).Six months after discharge,the self-efficacy of the intervention group was higher than that of the control group,the difference was statistically significant(P<0.05).(2)The two groups of family members of the intervention before the level of care and self-efficacy by two independent samples t test,the results showed: the difference was not statistically significant(P>0.05).After intervention,the care ability of the intervention group was higher than that of the control group at each measurement point(P<0.05).Repeated measurement variance analysis showed that the difference wasstatistically significant(P<0.001).Six months after discharge,the intervention group’s family self-efficacy level was higher than the control group,the difference was statistically significant(P<0.05).Conclusion:1.Home care needs of stroke patients are mainly affected by the ability of daily living activities,the after-effects and education level,stroke home care needs of family members affected by gender,marital status and the relationship between patients,therefore,this population of home care intervention should pay attention to the impact of these factors.2.Home care based on “Timing It Right” can improve the self-care ability of the young and middle-aged stroke patients and the care ability of their families and improve their self-efficacy.
