| Objective:To explore the vitamin D level and its correlation with non-alcoholic fatty liver disease among middle-aged and elderly individuals.Methods:Using the baseline data of"REACTION"study in Lanzhou,using the cluster random sampling method,select 3,237 residents of Lanzhou Wuquan Railway Xicun Sub-district,and extract general data questionnaire survey,physical examination,related biochemical testing and imaging data.Investigate the level of vitamin D,the prevalence and characteristics of lean non-alcoholic fatty liver disease(BMI<24.00 kg/m~2)/non-obese non-alcoholic fatty liver disease(BMI<28.00kg/m~2)/obese non-alcoholic fatty liver disease(BMI≥28.00 kg/m~2),and use Spearman rank correlation analysis was used to analyze the correlation between vitamin D level and NAFLD,and binary logistic regression was used to analyze the relationship between vitamin D and NAFLD risk.Results:(1)A total of 3038 study subjects were included,including 807 males(26.6%)and 2231 females(73.4%);age 40-75 years old,average(57.6±8.6)years old;average BMI was 24.24±3.16 kg/m~2;The average concentration of 25OHD was17.39±6.28 ng/ml,male(19.72±6.44 ng/ml)was significantly higher than female(16.54±6.00 ng/ml)(P<0.001).The vitamin D deficiency,deficiency,and adequacy rates in the entire population were 68.7%,28.5%,and 2.8%,respectively,and 97.2%of the survey subjects had vitamin deficiencies.The prevalence of vitamin D deficiency in women was significantly higher than that in men(73.91%vs54.03%,P<0.001).(2)The prevalence of lean/non-obese/obese-NAFLD in the respective BMI range of the population were 8.87%,20.50%,70.38%;the prevalence in the entire population was 4.31%,18.20%,7.90%.Among the lean,non-obese,and obese people,compared with the n-NAFLD group,the NAFLD group had higher physical measurement parameters,combined with more adverse metabolic factors,such as hypertension,diabetes,metabolic syndrome,and dyslipidemia.Compared with obese-NAFLD,the prevalence of hypertension in lean/non-obese-NAFLD population is lower(P<0.01).(3)Vitamin D levels are negatively correlated with overall NAFLD(r=-0.048,P=0.007).Vitamin D is also negatively correlated with non-obese-NAFLD(r=-0.041,P=0.034),and it is negatively correlated with lean/This correlation of obese-NAFLD disappeared(r=-0.019,P=0.465;r=-0.022,P=0.683).Binary logistic regression showed that after adjusting for age,waist circumference,TC,TG,HDL,LDL,hypertension,diabetes,snoring,smoking,and tea drinking,the high-level vitamin D group(Q4)was still better than the low-level vitamin D group(Q1).Reduce the risk of NAFLD by 25.2%(P=0.047)Conclusion:The prevalence of NAFLD is higher in the vitamin D deficiency group,and the serum vitamin D level is negatively correlated with NAFLD.Vitamin D supplementation may be an auxiliary way of NAFLD treatment. |
