Objective: To evaluate the efficacy and safety of 1927 nm Tm fiber fractional laser in the treatment of facial large pores.Methods: According to the inclusion and exclusion criteria,36 patients who presented with the main complaint of "large facial pores" to the Department of Dermatology,Affiliated Hospital of Guilin Medical College from January 2019 to December 2019 were screened out,and all subjects were treated with 1927 nm thulium fiber fractional laser for a total of 3 times,with an interval of 4 weeks between each treatment.Using the VISIA skin detector to take facial pictures and analysis,and using the high frequency ultrasound to measure the left cheek fixed point respectively 5 times,gather images,record and calculate the echo intensity value on average,before treatment and the 1,4,6 and12 weeks after the last treatment.Using SPSS22.0 and process the data to determine whether treatment effect is statistically significant.And the adverse reactions during the treatment were observed,and the satisfaction survey was completed by the patients.Results: 36 patients with large facial pore after treatment for 3 times,data analysis of VISIA skin detector showed that pore characteristics before treatment counts was 32.81±12.90,1 week after treatment,the pore characteristics of counting was 32.36±13.18,compared to prior treatment the difference was statistically significant(P < 0.05),4 weeks after treatment,the pore characteristics of counting was 25.71±11.56,compared to prior treatment the difference was statistically significant(P < 0.01),6 weeks after treatment and 12 weeks after treatment,the pore characteristics of counting was 18.48±7.33 and 17.95±6.96,compared to prior treatment the difference was statistically significant(P < 0.001).Also,the baseline UV spot feature count was 19.15±4.73,1 week after treatment UV feature count was 18.01±4.76,4 weeks after treatment and UV feature count was 14.40±4.37,6weeks after treatment and UV feature count was 13.03±3.87,12 weeks after treatment and UV feature count was 12.15±3.83,compared to prior treatment the differences were statistically significant(P<0.001);There were no significant differences in spots,red area,texture,wrinkles and brown spots at 1,4 and 12 weeks after treatment compared to prior treatment,or spots,texture,wrinkles and brown spots at 6 weeks after treatment compared to prior treatment(P>0.05).The prior treatment the red zone feature count was 34.13±10.97,and it was 36.80±12.34 at the 6th week after treatment,the difference was statistically significant compared to prior treatment(P<0.01).The high frequency ultrasound echo intensity data analysis showed that contrast echo intensity of 13.386±3.877 prior treatment,1 week after treatment the echo intensity 14.839±3.550,4 weeks after treatment the echo intensity 18.311±3.890,6 weeks after treatment the echo intensity 24.313±6.290,12 weeks after treatment the echo intensity 25.506±6.294,the differences were statistically significant compared to prior treatment(P<0.001).All 36 patients developed transient erythema edema after each treatment,which resolved within several to 72 hours.All 36 patients had mild scabs that fell off within 7 to 10 days.No severe adverse reactions were observed during the whole experiment.The minimum VAS score was 2,the maximum VAS score was 7,and the mean score was 3.72±1.56.In the satisfaction evaluation,the vast majority of people chose satisfaction or above,accounting for 94.4%.Conclusions: 1927 nm Tm fiber fractional laser can improve the coarse pores of facial skin,enhance the density of skin dermis,effectively brighten skin tone and improve superficial facial spots,with short recovery period and high patient satisfaction in the process.
