Purpose:I chose some sufferers of vulg aris acne with greasy facial skin and enlarged pores, whose body constitutions are classified as Damp-heat or phlegm type according to the Wang Qi’s Physical type nines, and observed their changes in facial grease and pore quantity after being treated with ALA-PDT. Having contrasted the differences in facial grease and pore quantity before and after treatment, i confirmed that ALA-PDT is effective at reducing facial grease and pore firming. Meanwhile, i studied the relationships between the two body constitutions and EYAN-PDT by comparing the two type of body constitutions. The results of all the studies have provided a convenient, practical and reliable basis for the clinic use EYAN-PDT.Method:1. Clinical observation:All the30cases were selected from30outpatients with Vulgaris Acne who came to Wuhan Hospital Dermatology from June2012to April2013and were selected with certain diagnostic standard and exclusion standard. we had selected patients the Damp-heat and Phlegm constitutions each15cases, Their body constitutions were diagnosed by the Dawson four diagnostic instrument,and all the patients are mild-to-moderate unusual type of acne patients according the international acne vulgaris three-four classifications.2. Therapy equipment:we spread in the face three hours with solution of3.6%5-aminolevulinic acid, and with LED-I photodynamic therapy machine as the light source,mostly irradiated with blue and yellow. The treatment was carried out once a week, and end up according to their conditions and degrees of treatment reaction.3. Efficacy:The patients were reviewed to evaluate the outcome of the treatment respectively1week,1month and3months after the end of the treatment.3.1The two groups of patients were assessed and scored by analyzing their facial grease and pores and with VISIA Skin Image Analyzer respectively1week,1month and3months after the treatment.3.2Three months later, after the third assessment, each of the30subjects made a self-assessment of the curable effects. The curable effects were assessed as3degrees:Very satisfied、Satisfied、 Dissatisfied. Then, a comparison between the total efficiency of both groups was made.3.3Besides VISIA,16of the30patients were also asked to use Derma Unit SSC3tester to measure the quantities of facial grease At the same time, Viva Scope1500confocal laser scanning microscope was also used to measure the diameters of the obviously visible enlarged the follicle month in these areas, and the depth is22.5um.4. Adverse Reaction:Record cases of dry that appeared in the Derma Unit SSC3tester results, then calculating the ratio. Furthermore, using the Tewameter TM300transepidermal test the loss of water.Results:1. The general situationThere were15cases in the Phlegm Group, among them,6cases finished one treatment,7cases finished two treatment,1cases f inished three treatment,1cases finished four treatment;There w ere also15cases in the Damp-heat Group, among them,3cases fin ished one treatment,8cases finished two treatment,3cases fini shed three treatment,1cases finished four treatment.2.VISIA scoresThe amount of oil secretion:The absolute score of the Phleg m Group before and1week,1month, and three month after the end of the treatment were respectively2.31±1.32、1.88±1.17.1.31±1.03、1.09±0.97. And the absolute score of the Damp-heat gro up were respectively3.12±0.40、1.14±0.27、0.61±0.16、1.15±0.46. Compared the two physical group, the p-value were respectivel y0.215、0.014、0.004、0.068(p<0.05has statistically significa nt).The number of visible pores in the unit area:The absolute sc ore of the Phlegm Group before and1week,1month, and three mon th after the end of the treatment were respectively1.03±0.21、1.16±0.13、1.05±0.09、0.84±0.11. And the absolute score of th e Damp-heat group were respectively1.42±0.14、0.98±0.15、0.93±0.13、0.49±0.03. Compared the two physical group, the p-value were respectively0.181、0.516、0.220、0.017(p<0.05has statist ically significant). 3.Derma Unit SSC3testerAmong the16patients, the cases that test result were oil before and1week,1month,and three month after the end of the treatment were respectively9,4,2,1at the forehead, respectively10,3,0,0at the double cheek, and respectively15,6,3,0in the lower jaw.4.The skin CTThe pore aperture of the16patients before and1week,1month, and three month after the end of the treatment were respectively0.31±0.17,0.25±0.56,0.19±0.11,0.17±0.01at the midpoint of the eyes and nose. And were respectively0.22±0.07,0.19±0.07,0.18±0.17,0.15±0.06at the midpoint of the eyes and mouth.5. Subject’s self-assessmentIn the30patients’ self-evaluation, the total satisfaction rate is up to96.9%, the Phlegm group satisfaction rate is93.3%, and the Damp-heat group satisfaction rate is100%. The efficiency of the Damp-heat group was significantly higher than the phlegm group.6.The adverse reactionsThe incidence of dry skin that occurred in Derma Unit SSC3tester test results before and1week,1month, and three month after the end of the treatment were respectively0、6.2%、3.1%、1.6%. The cases that epidermal moisture measurement result were dry before and1week,1month, and three month after the end of the treatment were respectively1,3,2,2at the forehead, respectively4,6,7,4at the double cheek, and respectively3,4,6,4in the lower jaw. The cases that transepidermal water loss results measured by Tewameter TM300tester were dry before and1week,1month, and three month after the end of the treatment were respectively0,1,4,1at the forehead, respectively1,2,4,2at the double cheek, and respectively 0,2,3,0in the lower jaw.Conelusion:1.Facial oil secretion and the number of visible pores all decreased with the increasing number of treatments, showing nega tive linear correlation.2. All the indicators confirmed that EYAN photodynamic thera py can be effective in improving acne patients’oily face and large pores. Compared with the Phlegm group, the Damp-heat grou p’s amount of the oil secretion and the number of visible pore s reduced faster. But with the passage of time, the gap between the two groups gradually narrowing especially by the time of t hree month after the treatment, there are differences between t he two groups no longer.3. EYAN photodynamic therapy will be accompanied by dry skin, skin moisture reduced and transepidermal water loss increased and other side effects, but the incidence is very low, and the sy mptoms can gradually ease at1mouth or3mouth after the end o f treatment.
