ObjectiveTo investigate the therapeutic effect of sequential therapy based on electrical stimulation acupoint therapy on chronic primary tinnitus and the possible factors affecting the therapeutic effect.Methods105 patients with chronic primary tinnitus were assigned to each group according to the random number table randomly.Group A was treated with Ginkgo tablet and Methylcobalamin capsule alone.On the basis of group A,group B was also treated with self-controlled non-invasive electrical stimulation acupoint therapy instrument.Group C was additionally treated with hormone for one week from the third week on the basis of group B.The treatment course was 1 month.Before and after treatment,SAS,SDS,SRSS,THI,VAS,TEQ were evaluated.The efficacy was evaluated,according to the evaluation of each scale before and after treatment and the follow-up at 1 month and 3months after treatment.Results1 The general condition of patients There were no statistically significant difference in the distribution of mean age,mean course of disease,acoustic immittance type,hearing curve typ,pitch grade,tinnitus side type and mean hearing threshold among the three groups,P > 0.05.2 Evaluation of each scale2.1 Comparison between groups:(1)Before treatment,there were no significant differences in THI,SDS,SAS,TEQ grade and VAS,SRSS score among the three groups,P>0.05.(2)After treatment,there were statistically significant differences in TEQ grade between group A and group C,and between group B and group C,P<0.05;There difference was no statistically significant in SRSS,VAS score and THI,SDS,SAS grade among the three groups,P>0.05.2.2 Intra-group comparison: there were statistically significant differences in TEQ,THI,SAS,SDS grade and SRSS,VAS score before and after treatment in each group,P<0.05.3 Analysis of curative effect and its influencing factors3.1 Efficacy: Group A,26 cases ineffective and 9 cases effective,the effective rate was25.7%.Group B,20 cases ineffective and 15 cases effective,the effective rate was42.7%.Group C,8 cases ineffective and 27 cases effective,the effective rate was77.1%.Chi-square test results: there were statistical differences in efficacy between group A and group C,and between group B and group C(P<0.017),that is,group C had the best efficacy,followed by group B,and group A had the lowest.3.2Analysis of the factors affecting the efficacy: there was no statistical significance in curative effect among patients with different age,gender,tinnitus side,acoustic immittance type,average hearing threshold,hearing curve type and tinnitus pitch frequency,P>0.05.4 Follow-up situation Group A,1 month after treatment,the effective rate was 22.9%;3 months after treatment,the effective rate was 20.7%.The effective rate of group B at 1 month and 3months after treatment was 42.7%.The effective rate of group C at 1 month and 3months after treatment was 77.1%.It can be seen that the treatment effect of group B and group C is stable,while the treatment effect of group a gradually decreases with the progress of time.ConclusionIt is safe and effective to treat chronic primary tinnitus with sequential therapy based on electrical acupoint stimulation,which has the value of further research and practice.In addition,the therapeutic effect was not related to gender,age,tinnitus side,acoustic immittance type,mean hearing threshold,hearing curve type and tinnitus pitch frequency.
