| Objective: To analyze the distribution and characteristics of TCM pneumonia with dyspneic cough syndromes of hospitalized children with respiratory syncytial virus pneumonia,so as to provide reference for TCM syndrome differentiation and treatment of children’s respiratory syncytial virus pneumonia,and further improve the clinical efficacy.Methods: Through self-designed questionnaire,323 cases of hospitalized children with respiratory syncytial virus pneumonia in our hospital from July 2020 to November 2020 were investigated by question answering and physical examination.The investigation items of TCM pneumonia with dyspneic cough syndrome include fever,heat peak,nasal congestion,runny nose,cough,expectoration,cough and vomiting,shortness of breath,lung wheezing,tongue quality,coating on the tongue,pulse condition(over 3 years old),infantile venule of index finger(under 3 years old),etc.Use Excel software to input data and establish a database,and use SPSS26.0 to statistically analyze the data.Results:1.The distribution of TCM syndromes was mainly phlegm-heat blocking the lung syndrome(197 cases,60.99%),followed by wind-cold blocking the lung syndrome(40cases,20.30%),Yin-deficiency and lung-heat syndrome(34 cases,10.53%)and wind-heat blocking the lung syndrome(32 cases,9.91%),toxic-heat blocking the lung syndrome(13cases,4.02%)and lung-spleen qi deficiency syndrome(7 cases,2.17%).The ages of children with various TCM syndromes are concentrated in the age range of ≥1 month to≤3 years old,and phlegm-heat blocking the lung syndrome was the main syndrome in all age groups.In addition to toxic-heat blocking the lung syndrome,the gender distribution of all TCM syndrome types was more boys than girls.2.The characteristics of each syndrome type: The main manifestations of wind-cold blocking the lung syndrome were fever,the heat peak is mainly moderate to high fever,nasal obstruction,runny nose and wet cough.The characteristics of tongue and pulse were pink tongue,thin and white coated tongue,floating red fingerprints.The main manifestations of wind-heat blocking the lung syndrome are fever,the heat peak is mainly moderate to high fever,nasal obstruction,runny nose and wet cough.The characteristics of tongue and pulse are light red or red tongue,thin and yellow coated tongue,light purple fingerprint.The main manifestations of phlegm-heat blocking the lung syndrome were fever,the heat peak is mainly moderate to high fever,nasal obstruction,runny nose and wet cough.The characteristics of tongue and pulse were red tongue,greasy coated tongue,light purple fingerprint.The pulse condition is mainly floating,slippery and counting.The main manifestations of Yin-deficiency and lung-heat syndrome are fever,the heat peak is mainly low fever,nasal obstruction,runny nose and cough.The characteristics of tongue and pulse are red tongue with little body fluid,little or thin and yellow coated tongue,and light purple fingerprint.The above four syndrome types can be identified from each other in the aspects of heat peak,cough,tongue texture,tongue coating and fingerprint.In addition,due to the small sample size and low statistical credibility,the syndrome of toxic-heat blocking the lung and the syndrome of lung-spleen qi deficiency are not distinguished from the other 4 syndrome types.However,this study found that toxic-heat blocking the lung syndrome can be distinguished from other syndromes in the aspects of heat peak,cough,shortness of breath,cough and vomiting,red and crimson tongue,thick and yellow coated tongue,deep purple fingerprint and the main position of the fingerprint is qi-guan.The lung-spleen qi deficiency syndrome has the potential to differentiate from other syndromes in the aspects of heat peak,cough,shortness of breath,audible wheezing sound in the lung,pink or pale white tongue,little or thin and white coated tongue,and fingerprint is reddish.Conclusion:1.This study found that the TCM syndrome type of hospitalized children with respiratory syncytial virus pneumonia is mainly based on the syndrome of phlegm-heat blocking the lung.The age of all TCM syndrome types of children are concentrated in ≥1 month to ≤ 3 years old,and all age groups are mainly phlegm-heat blocking the lung syndrome;in addition to toxic-heat blocking the lung syndrome,the gender distribution of all TCM syndrome types of children are more boys than girls;Therefore,it can provide a certain reference for the empirical treatment of children’s viral pneumonia by traditional Chinese medicine.2.This study shows that,in addition to the syndrome of toxic-heat blocking the lung and the syndrome of lung-spleen qi deficiency,which may cause low statistical credibility due to insufficient sample size,wind-cold blocking the lung syndrome syndrome,wind-heat blocking the lung syndrome,phlegm-heat blocking the lung syndrome and Yin-deficiency and lung-heat syndrome can be distinguished from each other in terms of heat peak,cough,tongue quality,tongue coating,and fingerprints.Therefore,it can provide a reference for the clinical identification of TCM syndromes of hospitalized children with respiratory syncytial virus pneumonia.And the reference basis also provides the basis of TCM syndrome characteristics for the treatment of the disease. |
