Objective: To investigate the consistency of pulse wave sphygmomanometer and mercury sphygmomanometer in blood pressure measurement.METHODS: A total of429 hypertensive patients, including 242 males and 187 females, aged between 45-75 years, who were treated in the outpatient and inpatient departments of cardiology and endocrinology of our hospital from August 2019 to June 2020 were selected. The blood pressure values of the patients were measured by pulse wave sphygmomanometer and mercury sphygmomanometer respectively. The values of systolic and diastolic blood pressure measured by the two kinds of sphygmomanometer were compared and analyzed. Results: In the unified comparison of 429 patients with mercury sphygmomanometer, the systolic blood pressure measured by the two kinds of sphygmomanometer were(136.20±23.48)mm Hg and(136.36±23.52)mm Hg,respectively, and there was no statistical difference between the two(P>0.05).Diastolic blood pressures were(78.25±13.69)mm Hg and(78.38±13.77)mm Hg,respectively, and there was no statistical difference between them(P>0.05).The Bland-Altman scatter plot was used to analyze the measurement results of systolic and diastolic blood pressure of the two kinds of sphygmograms. The results showed that the difference of blood pressure measured by the two kinds of sphygmograms was within the consistency limit of more than 95%.A total of 66 patients with systolic blood pressure ≥180mm Hg were divided into the high systolic blood pressure group, and 57 patients with diastolic blood pressure ≥110mm Hg were divided into the high diastolic blood pressure group. The results showed that the systolic blood pressure values of the two kinds of sphygmomanometers in the high systolic blood pressure group were(196.26±25.41)mm Hg and(198.80±23.72)mm Hg,respectively, and there was a statistical difference between the two groups(P< 0.05).In the high diastolic blood pressure group, the diastolic blood pressure measured by two kinds of sphygmomanometers were(127.25±17.64)mm Hg and(127.33±15.78)mm Hg,respectively, and there was no statistical difference between them(P>0.05).In addition,a total of 47 patients with significantly increased pulse pressure(SBP ≥80mm Hg)were separated and analyzed. The SBP measured by the two sphygmomanometers were(167.56±27.10)mm Hg and(168.83±26.92)mm Hg,respectively, and there was a statistical difference between the two(P<0.05).Diastolic blood pressures were(87.49±18.62)mm Hg and(87.55±18.98)mm Hg,respectively, and there was no statistical difference between them(P>0.05).62 patients with hypertension complicated with diabetes were divided into diabetes subgroup.Fifty-eight patients with hypertension and coronary heart disease were divided into coronary heart disease subgroup;Seventy-six patients with hypertension and simple atrial fibrillation were divided into atrial fibrillation subgroup. The results of numerical analysis of blood pressure measured by two kinds of sphygmomanometers in the three subgroups showed that the systolic blood pressure values measured by two kinds of sphygmomanometers in the diabetes subgroup were(134.68±24.52)mm Hg and(134.89±24.75)mm Hg,respectively, and there was no statistical difference between the two(P>0.05).The measured diastolic blood pressures were(80.50±11.95)mm Hg and(80.76±11.89)mm Hg,respectively, and there was no statistical difference between them(P>0.05).In the CHD subgroup, the systolic blood pressure values measured by two kinds of sphygmomanometers were(135.31±25.05)mm Hg and(136.86±25.23)mm Hg,respectively, and the diastolic blood pressure values were(77.48±11.24)mm Hg and(77.60±11.09)mm Hg,respectively, and there was no statistical difference between the two groups(P>0.05).The systolic blood pressure of the two kinds of sphygmomanometers in atrial fibrillation subgroup were(133.16±21.72)mm Hg and(136.67±21.65)mm Hg,respectively, and there was a statistical difference(P<0.05).Diastolic blood pressures were(65.30±14.21)mm Hg and(68.74±13.34)mm Hg,respectively, and there was a statistical difference(P<0.05).The Bland-Altman scatter plot was used to analyze the results of two kinds of sphygmograms in patients with atrial fibrillation. The results showed that in the comparison of systolic blood pressure, the percentage of points falling in 95% CI was about 15%.In the comparison of diastolic blood pressure, the proportion of external points falling in 95% CI was about 10%,indicating that the systolic and diastolic blood pressure measured by the two monitors had a low consistency within 95% confidence intervals. Conclusion: The measurement of systolic blood pressure and diastolic blood pressure of the two kinds of sphygmomanometer has a high consistency. When the systolic blood pressure is too high and the pulse pressure is obviously increased, there are differences in the measurement of systolic blood pressure between the two kinds of sphygmographs, and the consistency is low.In patients with simple hypertension complicated with atrial fibrillation, there are significant differences in the measurement of systolic and diastolic blood pressure between pulse wave sphygmomanometer and mercury sphygmomanometer, and there is a low consistency.
