Objective:Objective to explore the relationship between anxiety and depression,helicobacter pylori infection(Hp)and chronic atrophic gastritis.Namely,the influence of anxiety and/ or depression and Helicobacter pylori on the degree of gastric mucosal atrophy.When chronic atrophic gastritis is associated with Hp infection and anxiety and depression,on the basis of routine drug therapy,psychological intervention and related anti anxiety and depression drugs can reduce or even prevent the progression of the disease and improve the quality of life.Methods:From July2017toJ anuary2018,the first diagnosis of chronic atrophic gastritis and Hp test was performed in the outpatient department and outpatient department of the second hospital of Shanxi Medical University.Zung's anxiety self-rating scale(SAS)and depression self-rating scale(SDS)were used to investigate the patients,and the standard scores of SAS and SDS were calculated.The general data of the patients,the degree of atrophy of the gastric mucosal and the infection of Hp were collected.Results:Of the118chronic atrophic gastritis patients enrolled in this study,the average level of SDS was46.25points,the average level of SAS was47.5,and Hp was 55.1%,which was higher than that of the normal population in China.The level of anxiety and depression in female patients was significantly higher than that of male patients(P<0.05),but there was no correlation between age and anxiety and depression(P>0.05).No correlation was found between Hp infection and anxiety / depression level of patients(P>0.05).118 patients with chronic atrophic gastritis were divided into four groups according to whether they had anxiety/depression status and Hp infection(Double-positive group(Has anxiety/depression status and Hp infection),Simple Hp(+)group,Simple anxiety/depression(+)group,Double-negative group(Neither anxiety/depression nor Hp infection)).It was found that the degree of gastric mucosal atrophy was not equal between the four groups(P<0.001),and adjusted ? to0.008,we performed a pairwise comparison and found that: 1.The degree of gastric mucosal atrophy was significantly higher in the double-positive group than in the double-negative group(P<0.001).2.The degree of gastric mucosal atrophy was significantly higher in patients with simple Hp(+)group than in the double-negative group(P<0.001).3.The degree of gastric mucosal atrophy was significantly higher in the simple anxiety/depression(+)group than in the double-negative group(p<0.001).4.There was no difference in gastric mucosal atrophy between double-positive group and simple Hp(+)group,double-positive group and simple anxiety/depression(+)group,simple Hp(+)group and simple anxiety/depression(+)group(P>0.008).To further explore the risk factors leading to gastric mucosal atrophy and found: Anxiety and Hp infection are risk factors for gastric mucosal atrophy.For each degree of increase in anxiety,the risk of severe atrophy in the gastric mucosal increased by 9.591 times.Patients with positive Hp infection had a 12.563-fold increased risk of moderate to severe gastric mucosal atrophy.Conclusion:The level of anxiety/depression and Hp infection rate in patients with chronic atrophic gastritis are higher than those in the normal population,and women are more prone to anxiety/depression than men,but there is no correlation between gender and Hp infection rate.Patients with anxiety/depression status and/or Hp infection were more severe in gastric mucosal atrophy than those in the double-negative group.The proportion of patients with mild gastric mucosal atrophy in the double-positive group was the lowest,while the proportion of severe cases was the highest.It indicates that the gastric mucosal atrophy is more serious in the double-positive group.Although the results are not statistically significant,it is likely that meaningful results will be obtained if the sample size continues to expand.Both anxiety and Hp infection are risk factors for gastric mucosal atrophy,but the relationship between anxiety and depression levels and Hp infection has not yet been obtained.
