Objective:So far,stroke has become the first one in China disabled/fatal disease,cognitive dysfunction after stroke seriously affects patients’ daily life,the chronic non-communicable disease has seriously threatens people’s health and life security,timely and effective treatment can significantly slow the progress of the disease and promote health.Studies by scholars at home and abroad have proved that transcranial magnetic stimulation therapy and motion observation therapy have a positive effect on improving this dysfunction.The purpose of this study is to observe whether TMS combined with motor therapy is more effective in the treatment of cognitive impairment after stroke by analyzing the comparison of cognitive function of inpatients in our department from September 2018 to September 2019,so as to provide new ideas for clinical practice.Materials and methods:Subjects:the patients met the diagnostic criteria of the 4th Chinese academic conference on cerebrovascular diseases and were confirmed by head CT or MRI,with stable condition.The course of disease is less than one year;The score of the mini-mental State Examination(MMSE)was less than 24.Education in junior high school or above;Ages 18 to 65;Voluntarily participate in treatment and sign informed consent for rehabilitation.Grouping method:the 90 patients were divided into three groups according to different treatment methods,namely the conventional treatment group,the motion observation group and the combined treatment group.Evaluation indexes:patients’ Cognitive functions were compared before and after using the mini-mental State Examination(MMSE),Montreal Cognitive Assessment(MoCA)and P300.Statistical methods:SPSS22.0 statistical analysis package,continuity of data by a single sample of K-S normality test,accord with normal distribution of measurement data tox±S,its comparison with paired t test before and after each treatment group,quantitative data comparison between different groups using single factor analysis of variance(ANOVA),multiple comparisons using LSD method,P<0.05 judgment for the difference was statistically significant.Results:1.There were no significant differences in age,gender,disease course and years of education between the conventional treatment group,the action observation group and the combined treatment group(P>0.05).2.There was no significant difference in MMSE,MoCA score,amplitude and latency of P300 between the routine treatment group,the MoCA observation group and the combined treatment group before treatment(P>0.05).3.Age,gender,disease course and years of education had no significant influence on MMSE,MoCA scale score and amplitude latency of P300(P>0.05);4.Before and after treatment,the scale scores of the three groups increased,and the comparison results within the group showed that all scores in the MoCA scale increased significantly(p<0.05).The comparison results between the groups showed that there were significant differences in the increase of attention and total score before and after treatment(p<0.05).MMSE scores were improved,but not statistically significant(p>0.05).Before and after treatment,the amplitude of P300 cognitive potential was significantly increased(p<0.05),and the incubation period was significantly shortened(p<0.05).By comparing the three groups of patients before and after treatment,it was found that both the scale and P300 cognitive potential showed that the combined treatment group was superior to the action observation group and superior to the conventional treatment group.Conclusion:1.Transcranial magnetic stimulation(TMS),as a new therapeutic method,can significantly improve the cognitive function of patients after stroke.2.Transcranial magnetic stimulation therapy combined with action observation therapy based on mirror neuron theory has a better effect on improving the cognitive function of patients after stroke than action observation therapy,which can provide a new idea for clinical treatment.3.As a new treatment method,action observation therapy based on mirror neuron theory can significantly improve the cognitive function of patients after stroke.
