The Correlation Between Duration Of Untreated Psychosis Of First Episode Schizophrenia And Inflammatory Factors

Objective:Analysis of the correlation between inflammatory factors and the duration of untreated psychosis(DUP)in first-episode schizophrenic patients.Methods:1.To select the first-episode schizophrenic patients without taking any medicine and being in hospital in Yanbian Brain Hospital from Mar 2016 to Dec 2016 by using Nottingham Onset Schedule(NOS).A total of 84 cases were collected,and 57 patients were eligible for the study,of whom 35 were males and 22 were females.The median DUP for schizophrenic patients was 50W and the patients are divided into 29 cases of Short DUP groups(DUP<50 W)and 28 cases of Long DUP groups(DUP>50W).2.To evaluate mental pathological symptoms of schizophrenic patients by using Positive and Negative Syndrome Scale(PANSS).3.To gather the materials about the age,sex,marital status,height and weight of the patients.4.To take the blood(without eating anything for 12 hours)in the early morning;detect Homocysteine(HCY)by automatic biochemical analyzer;control the quality and classify by checking White blood cell(WBC),Red blood cell(RBC),Hemoglobin(HGB),Red blood cell volume(HCT),Platelet(PLT),Neutrophilic granulocyte(NEU%),Percentage of lymphocytes(LYM%),Neutrophil(NEU),Lymphocyte(LYM),Neutrophil-lymphocyte ratio(NLR)by automated hematology analyzer and supporting kits;calculate the NLR and detect the concentrations of Interleukin-6(IL-6)by using Enzyme-Linked Immunosorbent Assay(ELISA).5.The total data of this research are counted up and analyzed by statistical software SPSS 20.0.The measurement data of normal distribution with mean standard deviation(x�SD)said,t test for independent samples,non normal distribution of measurement data with median(P25,P75),Mann-Whitney U test;count data expressed as a percentage of chi square test;the correlation between DUP and inflammatory factors and symptoms of psychopathology,consistent with the normal distribution with Pearson correlation analysis,non normal distribution by using Spearman correlation analysis;inflammatory factors on DUP by logistic regression analysis,with P<0.05 as the difference was statistically significant.Results:1.In the short DUP group,8 cases were unmarried,married in 16 cases,divorced in 5 cases,the percentages were respectively of 27.59%,55.17%,17.24%.Long DUP group were 11 cases of unmarried,married in 6 cases,divorce in 11 cases,the percentage of the cases were respectively of 39.26%,21.48%,39.26%,the marital status distribution was statistically significant(X2=7.254,P<0.05)2.Short DUP group LYMPH%,NLR,HCY,PANSS total score,negative symptom score(NEGA),and IL-6 were respectively of 33.31?6.25,1.81?0.60,15.59�12.23,74.00�7.18,12.79�4.55 and 21.56(12.84,43.11),long DUP group LYMPH%,NLR,HCY,PANSS total score,NEGA,and IL-6 were respectively of 29.65�7.42,2.26�0.98,26.03�19.57,88.79�7.63,26.86�4.81 and 106.79(80.47,195.12),Long DUP group were higher than that of short DUP group,the differences were statistically significant(P<0.05)3.DUP was positively correlated with HCY,NLR,PANSS total score and NEGA in schizophrenic patients.The R values were respectively of 0.395,0.343,0.715 and 0.912,and the correlation was statistically significant(P<0.05).4.HCY was positively correlated with PANSS total score and NEGA in schizophrenic patients,the R value were respectively of 0.464,0.391,and the correlation was statistically significant(P<0.05).There was a positive correlation between NEGA and NLR,R value was 0.269,and the correlation was statistically significant(P<0.05).5.IL-6 was positively correlated with DUP,PANSS total score and NEGA in schizophrenic patients.The R values were respectively of 0.777?0.775 ? 0.912,and the correlation was statistically significant(P<0.05).6.Multivariate logistic regression analysis showed that IL-6 was closely related to DUP,and the OR value was 1.039(95%CI:1.017-1.061,P<0.05).Conclusion:In the first episode of schizophrenia,marital status is a factor of DUP.Inflammatory factors and psychopathology symptoms were different in patients with long DUP and short DUP.HCY,IL-6,and NLR may predict the course of schizophrenia,and the elevated IL-6 level is an independent risk factor for prolonged DUP.The levels of HCY,IL-6 and NLR can be used as a reference index to determine the severity of psychopathology in schizophrenia patients.
