Objective:To explore the influence and mutual relationship of duration of untreated psychosis on cognition,social function and BDNF in first-episode schizophrenia.Mathod:The study was conducted in the second affiliated hospital of jining medical college on the basis of informed consent.A total of 118inpatients who had not received previous therapy and whose conditions the first time were diagnosed with schizophrenia were included.Group A is stand for the 93 patients who can cooperate to fulfil the test of cognitive function within a week,group B is stand for the 93 patients who can cooperate to take on an empty stomach blood in the next morning,and group C is stand for the 68patients who can cooperate to not only fulfil the test of cognitive function within a week but aslo take on an empty stomach blood in the next morning.Group A and group B was respectively divided into the group with long DUP and the group with short DUP in each group DUP median split points.Group a or group b is respectivelystand for the 30 patients who were random drawn from group A or group B,at the same time,30 matched healthy worker of the hospital control subjects were included.Positive and Negative Syndrome Scale,GAF Global Assessment Function,Personal and Social Performance Scale,Matrics Consensus Cognitive Battery were conducted with the patients of group A.The surem BDNF concentrations of group B were measured by enzyme linked immunosorbent assay?ELISA?.Result:1.DUP of group A with first-episode schizophrenia showed a significant positive correlation with negative symptom score?r=0.393,P<0.05?and the general pathology score?r=0.212,P<0.05?;The scores of negative symptom of PANSS in long DUP group were higher than what in short DUP group,and there were significant differences in scores of PANSS and negative symptom of PANSS between the two group?P<0.05?.2.The scores of TMT?TMT,t=-4.110?,Symbol Coding test?SC,t=7.228?,Category Fluency test?CF,t=4.485?,stroop test?word t=7.240,color t=7.626,color-word t=4.485?,CPT?CPT,CPT1 t=7.618,CPT2 t=7.496,CPT3t=4.837?,Spatial Span test?SS,t=4.605?,HVLT-R?HVLT-R,HVLT-R1t=4.512,HVLT-R2t=6.679,HVLT-R3t=5.367?,BVMT-R?BVMT-R,BVMT-R1 t=3.127,BVMT-R2 t=3.495,BVMT-R3 t=4.113?and MAZES test?t=8.404?in group a with first-episode schizophrenia were all lower than what in matched group,and there were significant differences in scores of TMT,SymbolCoding test,Category Fluency test,stroop test,CPT,Spatial Span test,HVLT-R,BVMT-R and MAZES test between the two group?P<0.05?.In the group A,the scores of MSCEIT-TMTM test in long DUP group werelower than what in short DUP group,but there were no significant differences in scores ofMSCEIT-TMtest between the two group?t=1.635,P>0.05?.DUP of group A with first-episode schizophrenia showed a significant negative correlation with HVLT-R2 score?r=-0.265,P<0.05?and BVMT-R3score?r=-0.382,P<0.05?.In the group A,the scores of HVLT-R2 and BVMT-R3 inlong DUP group with first-episode schizophrenia were all lower than what in short DUP group,and there were significant differences in scores of HVLT-R2 and BVMT-R3 between the two group?P<0.05?.3.DUP of group A with first-episode schizophrenia showed a significant negative correlation with GAF score?r=-0.292,P<0.05?and PSP score?r=-0.397,P<0.05?;the scores of GAF and PSP in long DUP group were all lower than what in short DUP group,and there were significant differences in scores of GAF and PSP between the two group?P<0.05?.4.The surem brain-derived neurotrophic factor?BDNF?levels was significantly lower in patients of group b with first-episode schizophrenia compared tonormal controls[?10.33±4.68?ng/ml vs.?13.30±5.74?ng/ml,t=2.191,P<0.05].There were no significant correlation between DUP and the surem BDNF levels in patients of group B with first-episode schizophrenia,the surem BDNF levels was insignificantly higher in patients of short DUP group compared to long DUP group[?10.90±3.78?ng/ml vs.?10.25±4.72?ng/ml,t=0.733,P>0.05].5.The surem BDNF levels of first-episode schizophrenia showed a significant positive correlation in the score of MSCEIT-TM?r=0.335,P<0.05?.Conclusion:1.In the first-episode schizophrenia patients,the longer the DUP was,the worse the clinical symptoms,social function and cognitive function would be,especially negative symptoms of clinical symptoms,language learning and the visual learning ability of cognition even more significant.2.There was a wide range of cognitive function impairment in the first-episodeschizophreniapatients,includingspeedof processing,attention/vigilance,workingmemory,verballearning,visual learning,reasoning and problem solving ability.3.The surem BDNF was significantly downregulation in the first-episode schizophrenia patients compared to healthy controls,and DUP was not found to be associated with the surem BDNF in the first-episode schizophrenia patients.The higher the surem BDNF level was,the better the social cognition was.
