Objective:To detect the expression of CD200,IMP-3 proteins in the tissues of Classical Hogdkin’s lymphoma(CHL)patients by immunohistochemical assay.Collect the clinical indicators of CHL patients,compared the difference between IMP-3 and CD30,CD15 protein expression,and discuss the basis of IMP-3 as a diagnostic marker of CHL.To analyze the correlation between CD200 and clinical index,progression-free survival(PFS),overall survival(OS)of CHL patients,discuss the relationship between the CD200 protein on the prognosis of patients with CHL,provide a new idea for the diagnosis and treatment of classical Hodgkin’s lymphoma.Methods:A total of 48 cases of CHL patients diagnosed from 2009 to 2019 in the First People’s Hospital of Chenzhou were collected.Collect clinical data,tissue samples,and collect follow-up data of patients by telephone.The expression of CD200protein and IMP-3 protein in the diseased tissue was detected by immunohistochemistry.The results were interpreted by two pathologists according to the criteria.Data analysis using statistical analysis software SPSS26.0.Using Chi-square test to compare classified variables,Spearman test describes the correlation between continuous variables.Analysis of single factor survival using Kaplan-Meier method and draw the survival curve.Cox proportional risk model for multi-factor survival analysis.The test standard was set toα=0.05,P=0.05,the difference was statistically significant.Results:1、Immunohistochemical results showed CD200 are mainly expressed in HRS cell membrane and cytoplasm,with a positive rate of about 25%(12/48).The CD30 and CD15 were positive for the cytoplasm of HRS cells,CD30 positive rate reached 100%(48/48),CD15 positive rate was about 29.2%(14/48),PAX-5located in HRS cell nuclei,with a positive rate of about 87.5%(42/48).IMP3expressed in HRS cell cytoplasm,with a positive rate of about 95.8%(46/48).2、Using Chi-square test to analyze the difference of IMP-3 and CD15expression in CHL,the results showed that the difference between IMP-3 and CD15 groups was statistically significant(P<0.001),and the difference between the two groups was statistically significant(P<0.001).3、The Chi-square test analyzed the relationship between CD200 protein and clinical indicators,and the results showed that the difference of CD200 protein in sex was statistically significant(P=0.041).4、Spearman correlation test showed a correlation between LMR、NLR and PFS.The LMR was positively correlated with PFS(R=0.337,P<0.05),NLR was negatively correlated with PFS(R=-0.380,P<0.01).That is,when the LMR decreases and the NLR increases,the PFS shows a downward trend.5、Kaplan-Meier method was used for single factor analysis and survival curve was drawn.The results showed that the disease progressed faster when the number of lymph nodes involved was>3(P=0.022);ALC<0.6×10~9/L(P=0.004,P=0.026),male(P=0.030,P=0.004),with B symptoms(P=0.001,P=0.040),advanced stage(P=0.008,P=0.032),IPS-7 scores>2 points(P=0.033,P=0.001)and CD200 protein negative(P=0.048,P=0.002)had shorter progression-free survival and worse prognosis in patients with CHL.Of ALC(P=0.001,P=0.001),IPS-7 score>2 points(P=0.007,P=0.008),the difference of survival curve was statistically significant.6、The results of multivariate analysis showed that IPS-7 score(P=0.040,P=0.044)and ALC(P=0.022,P=0.022)were independent factors affecting the prognosis and progression of the disease.CD200 could not be used as an independent prognostic factor in CHL patients(P>0.05).Conclusion:1、The expression of IMP-3 can assist in the diagnosis of CHL.2、CD200 is associated with OS,PFS in CHL patients,but CD200 protein expression can not be used as an independent prognostic factor in CHL patients.
