Objective:classical Hodgkin Lymphoma is one of the most common malignant lymphomas, especially in Europe and North America. Recently, its frequency have the tendency of increasing in our country, mainly in young-adult. The pathogenesis is still not very clear. In domestic and abroad, the research on this disease mainly concentrate on genetic variation, markers of neoplastic cells, EB virus infection and microenvironment of tumor. The characteristic morphology of classical Hodgkin Lymphoma, rarity neoplastic cells and complex background cells, is one feature of the differences with other malignant lymphomas. Although many research suggest the neoplastic cells are origin from germinal center or post-germinal certer B-cell,it always not express B cell special markers.We observed the markers with neoplatic cells and background cells to approach the immunopathology characteristic.Material and methods:We collected 68 cases of diagnosed classical Hodgkin Lymphoma in pathology department of China-Japan union hospital of Jilin university,34 cases of inpatients,18 cases of outpatients,2 cases of consultation,14 cases of unlabeled; age between 7 and 75 years old, median age 26.5 years old, male to female ratio 1:1.13; all cases are from lymph nodes by biopsy,cutting tissue 61 cases,NS 17 cases, MC 24 cases, LR 17 cases and LD 3 cases, core needle biopsy tissue 7 cases,the tissue is too small to classify; 43 cases of neck,1 case of submaxillary,9 cases of clavicle,3 cases axillary fossa,4 cases of mediastinum,5 cases of abdomen,2 cases of inguen and 1 case of unnote. All antibodies used in these cases were brought from Maixin reagent company, including ALK,Bcl-2,Bcl-6,CD1a,CD3,CD4. CD10,CD15,CD20,CD21,CD23,CD30,CD45,CD45RO,CD68,CD79a,CK,LMP-1,EMA,Ki-67,Lysozyme,Melanoma,MUM-1,Pax-5,S-100 and Vimentin. We process data of positive rate of markers in neoplastic cells and background cells with SPSS 13.0 software using X2 test, P<0.05 consistent with statistical standard. Results:1)The positive rate of CD30 in neoplastic cells and background cells were 86.76%(59/68) and 20.59%(14/68):the positive rate of CD15 were 51.47%(35/68) and 63.24%(43/68).2)The neoplastic cells expressing B-cell immunophenotype were 29 cases, forming T-cell rings 10 cases; expressing T-cell immunophenotype were 31 cases, forming T-cell rings 20 cases (P>0.05).3) The expressing rate of Bcl-2 in neoplastic cells and background cells were 46.15%(24/52)和82.69% (43/52).while neoplastic express Bcl-2, it expressing B-cell and T-cell immunophenotype were 9 cases and 14 cases (P>0.05).4)The positive rate of LMP-1 was 22.03%(13/59).In cases of LMP-1 positive, the neoplastic cells expressing B-cell and T-cell immunophenotype were 7 cases and 3 cases (P>0.05). Conclusions:l)Both CD30 and CD15 are not specially to neoplastic cells in classical Hodgkin Lymphoma, they also express in background cells.2)The neoplastic cells expressing T-cell immunophenotype are higher than expressing B-cell immunophenotype in our collected cases. while forming T-cell rings, the neoplastic cells expressing B/T cell immunophenotype have no difference.3)Bcl-2 could express both in neoplastic cells and background cells. while the neoplastic cells expressing Bcl-2, they expressing B/T cell immunophenotype have no difference.4)LMP-1 protein of EBV only express in neoplastic cells and exist in the neoplastic cells that expressing B-cell and T-cell immunophenotype.
