| Objective: Through retrospective analysis of the clinical characteristics,survival and prognosis-related risk factors of children with malignant lymphoma diagnosed in the Second Hospital of Hebei Medical University from January 1,2015 to December 31,2019.Provide reference for diagnosis and treatment and prognosis evaluation.Methods:1.The 47 children with malignant lymphoma diagnosed in the Second Hospital of Hebei Medical University from January 1,2015 to December 31,2019 were selected as the research objects.Retrospective studies were used to analyze their clinical characteristics,and follow-up calls were used to understand the patients.Survival analysis of children,and conduct survival analysis,and analyze factors such as gender,age,pathological type,clinical stage,group B symptoms,liver and spleen involvement,bone marrow involvement,central nervous system involvement,giant tumors,albumin levels,LDH levels and other factors Statistical analysis to understand the influence of these factors on the prognosis of children.2.The statistical analysis was carried out by using SPSS 21.0 statistical software.The measurement data was expressed by rate(%),and the measurement data of non-normal distribution was expressed by the median.The survival characteristics were analyzed by Kaplan-Meier method,and the test level was α=0.05,P<0.05 is the difference is statistically significant,the log-rank test is used for univariate survival analysis,and the COX proportional hazard regression model is used for multivariate survival analysis.If P<0.05,the difference is statistically significant.Results: 1.From 2015 to 2019,47 cases of malignant lymphoma were diagnosed,including 5 cases,12 cases,5 cases,12 cases,and 13 cases respectively.2.Among the children diagnosed with lymphoma,there were 10 cases of Hodgkin’s lymphoma and 37 cases of non-Hodgkin’s lymphoma,the incidence ratio of the two was about 1:3.7.3.47 children with lymphoma,all ages of onset were < 14 years old,the youngest is 11 months old,the oldest is13 years old,the median age of onset of Hodgkin’s lymphoma and nonHodgkin’s lymphoma is 9 years old,Hodgkin’s lymphoma is in the 5-9 year old age group At most,5 cases(50%)are affected,and the maximum number of non-Hodgkin’s lymphomas is 21 cases(56.8%)in the age group of 10-14 years.4.There are 32 cases of male children,and 15 cases of female children.The incidence rate of males is higher than females regardless of Hodgkin’s lymphoma or non-Hodgkin’s lymphoma.The total male to female ratio is about 2.1:1.5.Hodgkin’s lymphoma or non-Hodgkin’s lymphoma.The common clinical manifestations of lymphoma were 8 cases(80%)of unexplained tumors,all of which were neck tumors.The clinical manifestations of non-Hodgkin’s lymphoma were diverse.16 cases(43.2%)were treated with unexplained tumors.There were 9 patients(24.3%)with digestive system symptoms,5 patients(13.5%)with systemic symptoms,3patients(8.1%)with respiratory symptoms as the main manifestation,and 3patients with bone and muscle pain as the first manifestation.Cases(8.1%),and 1 case(2.7%)had symptoms of the cranial five sense organs as the first manifestation.6.Common pathological types of Hodgkin’s lymphoma 21.2%,Burkitt,s lymphoma 29.8%,anaplastic large cell lymphoma 21.2%,T lymphoblastic lymphoma 12.8%,8.5% of unclear type,diffuse large B 4.3%of cell lymphoma,2.1% of 1 case of subcutaneous panniculitis-like T-cell lymphoma.7.The clinical stage of most cases has reached stage III or IV at the time of diagnosis,accounting for about 93.6%.The cumulative 5-year survival rate of 8.The 1-year cumulative survival rate of 47 children with malignant lymphoma was 83%,the 1-year cumulative event-free survival rate was 74.5%;the 5-year cumulative survival rate was 77.7%,and the 5-year cumulative event-free survival rate was 72.1%.9.Univariate analysis showed that liver and spleen involvement was a risk factor affecting the 5-year cumulative survival rate of children.Multi-factor analysis showed that the independent prognostic risk factors affecting the 5-year cumulative survival rate of children caused huge tumors,while gender,age,and pathology Type,clinical stage,group B symptoms,bone marrow involvement,central nervous system involvement,albumin level,LDH level,etc.are not risk factors that affect the survival rate of children.Conclusions:1.From January 1,2015 to December 31,2019,the number of confirmed cases of childhood malignant lymphoma in the Second Hospital of Hebei Medical University showed an overall upward trend.2.The incidence of Hodgkin’s lymphoma is lower than that of non-Hodgkin’s lymphoma.It is more common in male children,and the median age of onset is 9 years old.3.Children’s malignant lymphomas are mostly characterized by unexplained masses.Hodgkin’s lymphomas mostly invade the neck.In non-Hodgkin’s lymphomas,different pathological types have different onset locations.The common pathological type is Burkitt,s lymphoma,anaplastic large cell lymphoma,T lymphoblastic lymphoma,diffuse large B cell lymphoma,and most of them have reached stage III or IV at the time of diagnosis.4.The cumulative 1-year survival rate of children with malignant lymphoma diagnosed from January 1,2015 to December 31,2019 in the Second Hospital of Hebei Medical University was 83%,and the 1-year cumulative event-free survival rate was 74.5%;5-year cumulative The survival rate was 77.7%,and the 5-year cumulative event-free survival rate was 72.1%.5.The independent prognostic risk factor that affects the 5-year cumulative survival rate of children is huge tumor. |
