| Objective: To study the correlation of syndrome types of active rheumatoid arthritis with abnormal lipid metabolism in inpatients and outpatients of our hospital,and to explore the TCM syndrome types of active rheumatoid arthritis with abnormal lipid metabolism;to analyze the correlation between rheumatoid related indicators and blood lipid indicators,so as to provide some evidence and mentalities to contribute to clinical protection and treatment.Methods: 122 patients with active rheumatoid arthritis complicated with abnormal lipid metabolism in the ward and outpatient department of Affiliated Hospital of Shandong University of traditional Chinese medicine were selected,and the data were collected through inclusion and exclusion criteria,including: 1.General data: name,gender,age,address,contact information,hospitalization number(outpatient number),etc.;2.Clinical data: swelling joint number,tenderness,etc 3.Test indexes: ESR,Aso,RF,CRP,CCP,IgG,IgA,IgM,TG,TC,HDL,LDL,Apo A1,apo B,LP α.DAS28 score was calculated,TCM syndrome differentiation was given according to clinical symptoms,and entered into the database.Spss24.0 was used for data analysis.Results: 1.The main TCM syndromes of 122 cases were dampness heat syndrome(71 cases,58.20%),liver and kidney deficiency syndrome(19 cases,15.57%),phlegm stasis syndrome(18 cases,14.75%)and wind cold dampness syndrome(14 cases,11.48%).2.There were 97 female patients and 25 male patients,both of them were mainly damp heat syndrome.3.TCM Syndrome Types and age,number of tender joints,number of swollen joints,VAS,DAS28,CRP,CCP,RF,IgG,IgA,TC,HDL,LDL,Apo A1,LPα,Apo B,There was no statistical significance(P > 0.05);there was statistical difference with ESR level(P < 0.05),ESR level > wind cold dampness syndrome > phlegm and blood stasis syndrome > liver and kidney deficiency syndrome;there was statistical significance with ASO level,ASO level > dampness heat syndrome >phlegm and blood stasis syndrome > wind cold dampness syndrome;there was statistical difference with IgM level,IgM level of dampness heat syndrome >liver and kidney deficiency syndrome > phlegm stasis syndrome > wind cold dampness syndrome;contrasted to DAS28,the disease activity of patients with dampness heat obstruction syndrome was the highest > wind cold dampness obstruction syndrome > phlegm stasis obstruction syndrome > liver and kidney deficiency syndrome;The level of TG in patients with damp heat syndrome was higher than that in patients with phlegm and blood stasis syndrome,liver and kidney deficiency syndrome,wind cold and dampness syndrome.4.There was no correlation between RF,CCP and blood lipid;ESR had no correlation with TG,TC,LDL,apo B,LP α(P > 0.05),but had negative correlation with HDL(r =-0.306,P = 0.001)and Apo A1(r =-0.317,P = 0.000);CRP had no correlation with TG,TC,HDL,LDL,apo B(P > 0.05);and had negative correlation with Apo A1(r =-0.231,P = 0.011).5.There was no correlation between T28,sw28,VAS,DAS28 and blood lipid.Conclusion: 1.The incidence rate of female is higher than that of male.2.The levels of ESR,ASO,IgM,TG and DAS28 can help TCM syndrome differentiation to a certain extent.3.ESR was negatively correlated with HDL and Apo A1;CRP was negatively correlated with Apo A1. |
