| Objective: To observe the changes of the pressure wave and sound wave of the Cun pulse before and after treatment with Fu’s subcutaneous needling(FSN)in patients with neck-shoulder syndrome,and to explore the correlation between pain symptoms and the pressure wave and sound wave of the Cun pulse.Methods: Collected 50 patients with neck-shoulder syndrome from the Department of Traditional Chinese Medicine,Qianfoshan Hospital,Shandong Province,who suffered from right neck and shoulder pain,and were treated with FSN.Daosheng Four-diagnosis Apparatus(Model DS01-E)and Pulse Diagnosis Sound Wave Detection System were used to collect pressure waves and sound waves of the bilateral Cun pulse before and after treatment.SPSS 25.0 statistical software was used to compare the difference of pressure wave parameters before and after treatment,and analyzed the correlation between Northwick Park Neck Pain questionnaire(NPQ)score,Mcgill pain score and pressure wave parameters;Matlab 2019 b software was used to perform wavelet analysis,Hilbert-Huang Transform(HHT)analysis,Multi-Sample Entropy(MSE)analysis,spectrum and power spectrum analysis on the acoustic signal.Intuitive comparison to find positive waves of sound waves,compared the frequency difference of positive waves before and after treatment,analyzed the correlation between pain score and frequency of positive waves.Results:1.Evaluation of the efficacy of FSN treatment for neckshoulder syndrome: After once FSN treatment,the pain scores of PRI,VAS and PPI were all reduced,and the difference was statistically significant(P<0.001).2.Results of pressure wave of the right Cun pulse:(1)Before treatment,NPQ score was positively correlated with h5/h1 and negatively correlated with h4;PRI sensory item score was positively correlated with h5/h1,h5;PRI emotional item score was positively correlated with t1 and h5/ h1,and negatively correlated with h3,h4 and As;PRI total score was positively correlated with h5/h1,and negatively correlated with h4;VAS score was positively correlated with h5/h1,and was negatively correlated with h1,h2,h3,h4,As,Ad and A;PPI score was positively correlated with h5/h1,and negatively correlated with h3,h4,and h4/h1,which is statistically significant(P<0.05).(2)After treatment,t5 and h4/h1 was increased,h5,h5/h1,and pulse frequency were decreased,which was statistically significant(P<0.05).(3)After treatment,the PRI sensory item score was negatively correlated with h4,h4/h1;the PRI emotional item score was negatively correlated with h1,h2,h3,h4,As,Ad,and A;the PRI total score was negatively correlated with h1,h3,h4,h4/h1,As,Ad,A,t5/t4 were negatively correlated;VAS score was positively correlated with h5/h1,h5,and negatively correlated with t4,h3/h1,h2,h3,h4,h4/h1,and As;PPI score was positively correlated with h5/h1,and negatively correlated with t4,h3/h1,h2,h3,h4,h4/h1,As,Ad,A,and t,with statistical significance(P<0.05).(4)The difference of VAS score before and after treatment was positively correlated with the difference of h4/h1,and the difference of PPI score was negatively correlated with the difference of t1/t,which was statistically significant(P<0.05).4.Results of sound waves in Cun pulse:(1)Through intuitive comparison of sound wave waveforms in time domain,wavelet analysis,and HHT analysis,positive sound waves can be identified.(2)The MSE values of the 1-5layers of the right Cun pulse and the 3-4 layers of the left Cun pulse decreased after treatment,and the difference was statistically significant(P<0.05).There was no significant change in the MSE value of left Cun 1-2 layer after treatment(P>0.05).(3)The frequency of positive sound waves in the 1-5 layers of the bilateral Cun pulse decreased after treatment,and the difference was statistically significant(P<0.05).(4)The frequency of positive waves in the first layer of the right Cun pulse was positively correlated with NPQ score,VAS score,and PPI score,and the frequency of positive waves in the second layer of right Cun pulse was positively correlated with NPQ score,PRI emotional item score,PRI total score,VAS score,and PPI score.The frequency of positive waves in the third layer and the fourth layer of the left Cun pulse positively correlated with the NPQ score,and the frequency of the positive waves in the fifth layer of the right Cun pulse positively correlated with the NPQ score and the PPI score,with statistical significance(P<0.05).Among them,the correlation coefficient r with NPQ score was sorted from big to small as:the first layer of the right Cun pulse> the second layer of the right Cun pulse> the fifth layer of the right Cun pulse> the third layer of the left Cun pulse> the fourth layer of the left Cun pulse.(5)The difference in frequency of positive waves in the second layer of right Cun pulse was positively correlated with the difference in pain symptom scores,which was statistically significant(P<0.05).Conclusion: 1.FSN treatment significantly relieves the pain symptoms of patients with neck-shoulder syndrome.2.FSN therapy has a certain impact on the pressure wave parameters of the Cun pulse in patients with neckshoulder syndrome,reducing the frequency of positive sound waves and reducing the complexity of the acoustic signal in Cun pulse.3.Pain symptom scores and differences before and after treatment are related to some pressure wave parameters and the frequency of positive waves.The frequency of positive waves in the first layer of the right Cun pulse can best reflect the severity of the patient’s neck and shoulder pain.The frequency difference of the positive wave of the layer acoustic wave can reflect the degree of pain relief of floating needle treatment. |
