Clinical Application And Research Based On The Real-time Two-dimensional Shear Wave Elastography In The Therapeutic Evaluation Of Neck-shoulder Myofascial Pain Syndrome By Acupuncture Treatment

Objective: 1.In order to evaluate the normal upper trapezius stiffness of the neck,the shear-wave elastrography is applied.The range of normal upper trapezius stiffness is obtained according the research.2.Evaluation of Young's modulus values of myofascial neck shoulder syndrome patients before and after acupuncture treatment are made and compared based on SWE.The difference of the young's modulus of upper trapezius of the neck at different time points is observed,and the clinical application based on SWE in the therapeutic evaluation of neck-shoulder myofascial pain syndrome by acupuncture treatment is preliminary discussed and analyzed.Method: 1.The normal people who was received physical examination from February,2016 to December,2017,a total of 98 people are selected as the normal group.The Doppler color ultrasonic diagnostic machine(Aix-plorer type)of French Supersonic Imagine Company is used to obtain ultrasound image of bilateral upper trapezius paramagnetic fiber of normal group in the mode of shear wave elastography.2.The 60 events,who was diagnosed as neck-shoulder myofascial pain syndrome from February,2016 to December,2017,are selected as the research object.These are randomly divided into two groups,which are simple acupuncture group and acupuncture electro acupuncture group,and received acupuncture treatment of traumatology.The elasticity of upper trapezius are measured at different time points,which are before therapy and immediately after therapy,2 week and 4 weeks after therapy,According to Visual Analogue Scale(VAS)Pain,pain intensity and Neck Disabilitv Index(NDI)scale table,the correlation analysis between the elasticity values and therapeutic effectiveness ofneck-shoulder myofascial pain syndrome before and after acupuncture treatment are conducted.Results: 1.The results show that there is a linear negative relationship between the elasticity values and the ages for the normal group(P<0.001),the difference of mean value,maximum value and minimum values of upper trapezius for normal group between male and female is small,which shows no statistical differences from the perspective of gender differences.2.According to the comparison of statistical value of Young's modulus of upper trapeziuses including Mean?Max?Min values at different time points for two groups,there is no statistical significance difference between the values and groups,besides,there is no statistical significance difference between the value of elasticity values of upper trapeziuses and gender(P>0.05).The statistical value of elasticity values(Mean,Min,Max)is negative linear correlate with age both in the two groups.There are significant statistical differences in the values of elasticity values at different time points both in the two groups(P < 0.05).However,there are no statistical differences of the value of elasticity values before and after acupuncture treatment for both groups,and also there are no statistical differences in the therapeutic evaluation before and after acupuncture treatment for both groups.There are statistical differences in the value of elasticity values of upper trapeziuses before and after acupuncture treatment between two groups and also in the therapeutic evaluation between two groups.Conclusion: 1.The results indicate that there are no statistical differences for the muscle elasticity of upper trapezius from the perspective of gender differences.there is a negative linear relationship between muscle elasticity of upper trapezius and the ages for the normal group.the reference range of elasticity values of upper trapezius for normal group with 95 CI is 18.17-33.49 k Pa.2.According to the statistical data obtained by the treatment of neck-shoulder myofascial pain syndrome through two methods,here are no statistical differences in gender and age in both two groups.And there is no statistical difference in the index of therapeutic evaluation including VAS,PPI and NDI scale table before and after acupuncture treatment for both methods,which indicated the there is no difference between the two methods.At the aspect of therapeutic evaluation,it is shown some conclusions from the correlation analysis between the E values of elasticity values of upper trapeziuses and the index of VAS,PPI and NDI scale table:there is a linear relationship between the E values and the index of VAS,PPI and NDI scale table,There is a significant reduction of the values of elasticity values of upper trapeziuses after acupuncture treatment,which is in accordance with the evaluation of neck function after acupuncture treatment.
