| This study includes two parts:experimental study and clinical study.The experimental study is to explore the mechanism of Xiongzhi Zhentong Decoction in the treatment of migraine.The clinical study is to observe the clinical efficacy of Xiongzhi Zhentong Decoction on blood stasis blocking collaterals Migraine.The study is to provide theoretical basis for clinical application of X iongzhi Zhentong Decoction in the treatment of migraine.Experimental study:Objective:The purpose is to observe the effect of Xiongzhi Zhentong Decoction on the number of head scratching in migraine rats and on TRPV1 expression of trigeminal ganglion.Methods:We divided the SD rats into 6 groups at random:blank group,model group,Xibiling group,low,medium and high dose group of Xiongzhi Zhentong Decoction.Rats were given continuous gavage for 7 days,1 per day.We used the equal volume saline to drench the blank group and the model group.After half an hour of last gavage in each group except for blank group,each group was injected with nitroglycerin injection at 10mg/kg in neck in preparation for migraine rats model.After successful modeling,the number of head scratches was recorded according to 0-30min、30-60min、60-90min、90-120min.Take out the trigeminal ganglion of rats and then use immunohistochemical method to detect the TRPV1 expression of trigeminal ganglion.Results:1.Effect of Xiongzhi Zhentong Decoction on Head Scratching Number in Migraine RatsBy contrast with the model group,the number of head scratching in the low,medium and high dose group of Xiongzhi Zhentong Decoction and Xibiling group was decreased in each time period.The difference was statistically significant(P<0.05 or P<0.01),showing that the low,medium and high dose of Xiongzhi Zhentong Decoction and Xibiling improved the behavioral symptoms of migraine rats.The number of head scratching in rats with low,medium and high dose of Xiongzhi Zhentong Decoction was lower than that of Xibiling in 30-60min、60-90min、90-120min(P<0.05 or P<0.01),showing that the effect of low,medium and high dose of Xiongzhi Zhentong Decoction on behavioral symptoms of migraine rats was better than that of Xibiling.2.Effect of Xiongzhi Zhentong Decoction on TRPV1 Expression of Trigeminal Ganglion in Migraine RatsA small amount of TRPV1 was expressed in trigeminal ganglion of rats in blank group.By contrast with the blank group,the TRPV1 mean optical density was increased in the model group,Xibiling group,low,medium and high dose group of Xiongzhi Zhentong Decoction(P<0.05),showing that nitroglycerin can increase TRPV1 expression and TRPV1 can induce aseptic neurogenic inflammation,then to produce migraine.By contrast with model group,the TRPV1 mean optical density in rats with low,medium and high dose group of Xiongzhi Zhentong Decoction and Xibiling group was decreased(P<0.05),showing that Xiongzhi Zhentong Decoction and Xibiling can inhibit the expression of TRPV1 in migraine rats to reduce aseptic neurogenic inflammation for treating migraine.By contrast with Xibiling group,the TRPV1 mean optical density in high dose group was decreased(P<0.05),showing that the effect of high dose of Xiongzhi Zhentong Decoction on TRPV1 expression was better than that of Xibiling.By contrast with low and medium dose,the TRPV1 mean optical density in high dose group was decreased(P<0.05),showing that high dose of Xiongzhi Zhentong Decoction had better effect on TRPV1 expression than low and medium dose.There was no statistical difference between low and middle dose,showing that effect of low and medium dose of Xiongzhi Zhentong Decoction on TRPV1 expression was equal.Conclusion:Xiongzhi Zhentong Decoction can reduce the number of head scratching and improve behavioural symptoms in migraine rats.Xiongzhi Zhentong Decoction can reduce the TRPV1 expression of trigeminal ganglion in migraine rats,better at high dose.Xiongzhi Zhentong Decoction may reduce aseptic neurogenic inflammation by inhibiting the TRPV1 expression of trigeminal ganglion to treat migraine.Clinical study:Objective:The purpose is to discuss the clinical effect of Xiongzhi Zhentong Decoction on treating the blood stasis blocking collaterals migraine.Methods:60 patients with blood stasis blocking collaterals migraine were divided into two groups at random:treatment group 30 patients,control group 30 patients.Xibiling(flunarizine hydrochloride capsule)was used in the control group.Xiongzhi Zhentong Decoction was used in the treatment group.The two groups were treated continuously for 2 weeks as a course of treatment.2 weeks after treatment,the two groups received follow-up visit.The patients in both groups were evaluated with clinical symptoms and signs scale of migraine and TCM syndrome scale before treatment,at the end of treatment and two weeks after treatment.Results:1.Comparison of Clinical Symptoms and Signs Points after TreatmentPoints of clinical symptoms and signs showed points before treatment>end of treatment>two weeks after treatment.The points were constantly decreased,suggesting that the treatment of Xiongzhi Zhentong Decoction and Xibiling can relieve the clinical symptoms of migraine patients.At the end of treatment and 2 weeks after treatment,the integral of the treatment group was apparently smaller than that of the control group(P<0.01),showing that at the end of treatment and 2 weeks after treatment,the therapeutic effect of Xiongzhi Zhentong Decoction was significantly better than that of Xibiling.2.Comparison of Clinical Symptoms and Signs Grade after TreatmentAt the end of treatment and 2 weeks after treatment,the grade of both groups was decreased.The grade of treatment group decreased apparently than that of control group(P<0.05),showing that the curative effect of Xiongzhi Zhentong Decoction at the end of treatment and 2 weeks after treatment was better than that of Xibiling.3.Comparison of TCM Syndrome Points after TreatmentPoints of TCM syndrome showed points before treatment>end of treatment>two weeks after treatment.The points were constantly decreased,suggesting that both Xiongzhi Zhentong Decoction and Xibiling have good clinical effect.At the end of treatment and 2 weeks after treatment,the TCM syndrome score of treatment group was lower than that of control group(P<0.05),showing that the clinical effect of the treatment group was better than that of the Xibiling.4.Comparison of the Efficacy of TCM Syndrome PointsAt the end of treatment,the total effective rate was 80%in the treatment group and 50%in the control group.Two weeks after treatment,the total effective rate was 96.67%in the treatment group and 66.67%in the control group.The total efficiency of the treatment group was higher than that of the control group at the end of treatment and two weeks after treatment(P<0.05),showing that at the end of the treatment and 2 weeks after treatment,the curative effect of Xiongzhi Zhentong Decoction was better than that of Xibiling.Conclusion:Both Xiongzhi Zhentong Decoction and Xibiling have the curative effect of treating migraine and can reduce migraine clinical symptoms and signs points,TCM syndrome points.But Xiongzhi Zhentong Decoction has better clinical effect and it deserves further clinical popularization and application.
