Clinical Observation Of Jianguling Decoction Combined With Percutaneous Pin Fixation In The Treatment Of Distal Radius Fractures

Objective: Objective to explore the clinical effect of Jianguling decoction combined with percutaneous pin fixation in the treatment of distal radius fractures.Methods: from November 2018 to December 2020,60 cases of osteoporotic distal radius fractures aged 55-80 years old in the Department of orthopedics,Affiliated Hospital of Shandong University of traditional Chinese medicine were selected and randomly divided into group A(observation group)and group B(control group).Group A was treated with percutaneous pin fixation combined with Jianguling decoction,and group B was treated with percutaneous pin fixation,with 30 cases in each group.Both groups were treated with Xuefu Zhuyu oral liquid for promoting blood circulation and removing blood stasis,detumescence and pain relief before and 2 weeks after operation.The patients in group A were treated with Jianguling decoction after operation,such as tonifying liver and kidney,helping Yang and removing dampness,strengthening tendons and bones,promoting blood circulation and removing blood stasis.The prescription is mainly composed of Psoralea corylifolia,Eucommia ulmoides,Drynaria,Dipsacus,epimedium,Cnidium monnieri,Achyranthes bidentata,safflower,radix rehmanniae,etc.1 dose / D,decoction,morning and evening warm,take for 3 months.Two groups of patients were reexamined once a week,lasting for 4 weeks.The operation plan should be determined according to the actual physical condition of the patients,and try to meet the needs of the patients as much as possible,and strive to achieve the best postoperative effect.Visit for 3months,and take photos 15 days,1 month,2 months and 3 months after the operation to observe the fracture healing.Telephone follow-up observation was performed to record the swelling time of wrist joint and the clinical healing time of fracture;At 3 months after operation,palmar inclination,ulnar deviation and radial height were measured;The VAS pain score at different time points,Gartland and werley(G-W)wrist score at 3 months after operation,postoperative complications,etc.were statistically analyzed by spss26.0 to provide strong data support for the advantages of modified Jianguling formula combined with percutaneous pin fixation in the treatment of osteoporotic distal radius fractures,Objective to provide a reliable clinical basis for the treatment of osteoporotic distal radius fractures with modified Jianguling formula combined with percutaneous pin fixation.Results: During the follow-up,there were 3 patients in the observation group(group A)and 2 patients in the control group(group B).The rest of the patients were followed up for 3 months.Statistical analysis showed that there was no significant difference in preoperative basic data between the two groups(P > 0.05).There were significant differences in swelling time of wrist joint,VAS pain score after operation(14 days,1month,3 months),fracture healing time,palmar inclination,ulnar deviation angle,Gartland and werley(G-W)wrist score between the observation group and the control group(P < 0.05),The results showed that the observation group was superior to the control group in clinical effect.Conclusion: Jianguling decoction combined with percutaneous pin fixation plays a positive role in postoperative rehabilitation of patients with osteoporotic distal radius fracture,which can effectively reduce pain,promote wrist joint swelling and fracture healing,and improve the quality of life of patients after operation,which should be developed,innovated and applied to clinical treatment.
