Prof Yitao Xue’s Drug Rule Analysis And Preliminary Discussion On The Mechanism Of Treating Heart Failure

ObjectiveThe first-visit medical records of patients with heart failure(HF)in the outpatient treatment by the tutor,Professor Yitao Xue,were collected and analyzed by combining with the data mining method of the traditional Chinese medicine inheritance auxiliary platform.Then,the rules and prescription characteristics of Professor Yitao Xue’s treatment of HF with traditional Chinese medicines were discussed to provide better and more effective ideas and methods for the treatment of HF patients.Based on Professor Yitao Xue’s medication rules,an animal model of HF induced by Adriamycin(ADR)was established to analyze the pathological changes of myocardial tissue in the process of HF;the molecular mechanism of the development and change of HF after the intervention of the three core Traditional Chinese Medicine of Aconite,Astragalus,and Tinglizi in Fuxin Mixture was explored.The results provide a theoretical basis for the clinical treatment of HF.Methods1.Prof.Yitao Xue’s analysis of clinical medication rules in the treatment of HFThe medical records of the outpatient treatment of heart HF by Professor Yitao Xue at the Affiliated Hospital of Shandong University of Traditional Chinese Medicine from March 2016 to March 2021 were collected and organized.The first-diagnosed Chinese medicine prescriptions were collected as the research object and logged into the"Traditional Chinese Medicine Inheritance Assistant Platform(V2.5)" software.Based on the "Traditional Chinese Medicine Inheritance Assistant Platform" software,the commonly used core drugs in all prescriptions were excavated,and the academic thoughts of Professor Yitao Xue were summarized.2.Explore the molecular mechanism of core Traditional Chinese Medicine Aconite,Astragalus,and Tinglizi in the treatment of HFNinety 8-week-old male C57BL/6 mice were randomly divided into a normal control(NC)group,a heart failure model(MOD)group,a high-dose Chinese medicine group(FXM-L),a low-dose Chinese medicine group(FXM-H)group,an astragaloside Ⅳ(AS-IV)group,and a trimetazidine(TMZ)group.The HF model was established by intraperitoneal injection of adriamycin.The core traditional Chinese medicine high-dose and low-dose aqueous solutions,astragaloside Ⅳ,and trimetazidine hydrochloride were used for drug intervention.Six weeks after the model was created,the myocardial tissue was removed by anesthesia.Afterward,the content of adenosine triphosphate(ATP)in myocardial tissue was detected,and the expression and activity of Na+/K+-ATPase in myocardial tissue were observed.Additionally,the changes of potassium ion(K+)flow rate in the myocardial tissue of C57BL/6 mice in each group 6 weeks after modeling were monitored.Results1.As revealed by the statistical analysis of the collected medical records of 134 first-time visiting patients,the total frequency of each Chinese medicine in all prescriptions is higher than that of Astragalus,Amomum villosum,Angelica,Aconite,Tinglizi;the drugs are mild drugs,accounting for 41.42%,followed by cold drugs,calm drugs,hot drugs,and cool drugs;the five flavors were mainly sweet,accounting for 41.95%,followed by pungent,bitter,sour,salty,and astringent;the meridian was based on the spleen,lung,and heart;a total of 12 potential core drug combinations and 6 potential new prescriptions were extracted using data mining on the auxiliary platform.2.After successful modeling,the pathological results demonstrated that the myocardial tissue of the C57BL/6 mice in the NC group was uniformly stained,and the myocardial fiber morphology and structure were normal,with clear boundaries and regular arrangement.There was no significant abnormality in the interstitium and no obvious inflammation.In the MOD group,there were considerable myocardial fibers dissolving and breaking,a small amount of myocardial fiber vacuole degeneration,round vacuoles of various sizes,and eosinophils.The treatment groups of FXM-H,FXM-L,AS-IV,and TMZ all had different degrees of myocardial fiber dissolution and rupture.Compared with the NC group,the ATP content and Na+/K+-ATPase expression and activity in the myocardial tissue of the MOD group were significantly reduced;K+homeostasis in the cell was disrupted.Compared with the model group,the ATP content and the expression and activity of Na+/K+-ATPase in myocardial tissue of mice were significantly increased in the FXM-H,FXM-L,AS-IV,and TMZ groups.ConclusionsBased on the analysis of the traditional Chinese medicine inheritance auxiliary platform,Professor Yitao Xue’s prescription and medication rules for the treatment of HF were analyzed.The treatment of HF is mainly to benefit the temperature and yang,promoting blood circulation and diuresis.The the three core Traditional Chinese Medicine of Aconite,Astragalus,and Tinglizi in Fuxin Mixture can effectively increase the expression of ATP1a1 in the myocardial tissue of mice with HF,enhance the level and activity of Na+/K+-ATPase,increase the content of ATP,maintain K+homeostasis inside and outside the cell,improve the energy metabolism of the heart,and correct HF.In this study,the academic thoughts and clinical experience of famous Chinese medicine practitioners were explored to lay a foundation for the clinical application of Chinese medicine in the treatment of HF.
