Disscussion On Professor Wang Jingqing’s Experience In Diagnosis And Treatment Of Insomnia From The Perspective Of Heart-kidney Incompatibility

Purpose:To sort out some of the medical records of Professor Wang Jingqing’s outpatient department from the perspective of "heart-kidney incompatibility" in the diagnosis and treatment of insomnia,combine modern information technology methods,use data mining technology to process and analyze the data,and summarize the relevant experience and academic thoughts of the tutor in the treatment of insomnia with expectation of providing new ideas for clinical treatment of insomnia and provide a certain basis for further research about insomnia.Method:Firstly,collect and summarize some of the insomnia medical records of Director Wang Jingqing from April 2017 to January 2021,conduct preliminary screening,then diagnose and establish the information of 120 eligible insomnia cases which were treated from the perspective of "heart-kidney incompatibility" in a database,and after the data is processed,the data processing platform Medcase V3.8 data record mining system is used for data analysis and visual expression.To analyse and summarize Professor Wang Jingqing’s experience in diagnosis and treatment from the perspective of "heart-kidney incompatibility from the perspective of clinical manifestations,tongue coating,pulse conditions,pathogenesis,and prescription medication rules.Results:1.Age distribution:50-59 years old patients accounts for the most(35%),followed by 40-49(30.83%)and 60-69(16.67%).It also shows that the 50-59 age group is the peak period of disease.2.Clinical manifestations:The main symptoms of the top three frequencies are difficulty in falling asleep,easily getting awake and more dreams,and the accompanying symptoms of the top three frequencies are sweating,dizziness and anxiety.3.Tongue condition and pulse condition:The main manifestations of the tongue quality are red,light red,and dark red.The most common is red.The main manifestations of tongue coating are thin yellow coating,yellow greasy coating,and little coating.The most common is thin yellow coating.The first three pulse types with pulse frequency are Huamai,Ximai,Xuanmai,and the frequencies are 30.38%,24.23%,and 19.62%,respectively.4.Pathogenesis:A total of 26 pathogenesis is involved,among which the main pathogenesis is the failure of heart and kidney,the second is phlegm heat disturbing the heart;the second is Yin deficiency and spleen deficiency.5.Governing rules and treatment methods:there are 28 treatment rules and treatment methods involved,of which the main treatment methods are to communicate the heart and kidney,resolve phlegm,calm the nerves,and clear away heat.It is often used in combination with the treatment of phlegm,clearing away heat,soothing the nerves,strengthening the spleen,and calming the heart.6.There are four main high-frequency drugs with a frequency of use greater than 5%,including Polygala,Shouwu Veng,Fushen,and Coptis.7.The data after the application of the association rules show:Symptoms:When the patient has difficulty falling asleep,it is often accompanied by sweating,dreaminess,anxiety,etc.Tongue condition:thin white coating,light red texture,thin yellow coating and red texture,pulse condition:pulse is dense and thin,and pulse is thin and slippery.Pathogenesis:phlegm heat disturbs the heart,stomach disharmony,liver depression and spleen deficiency are highly correlated with heart-kidney incompatibility.Therapeutic principle:high correlation with stomach,lowering fire,soothing liver,nourishing yin and communicating heart and kidney.Medication:Contains 6 drug combinations with high correlation,namely:Fushen+Coptis,Fushen+Shouwu Veng,Fushen+Polygala,Coptis+Polygala,Coptis+Shouwu Veng,Shouwu Vine+Polygala.8 After using the cluster analysis algorithm,it can be seen that the commonly used drug pairs are:Fushen+Coptis,Hawthorn+Liushenqu,Gardenia+Cyperus rotundus,Fried Atractylodes macrocephala+Pseudoginseng and so on.The core prescriptions are:Rhizoma Coptidis,Fushen,Polygala,Shouwu Veng.Conclusion:Professor Wang Jingqing diagnoses and treats insomnia from the perspective of"heart-kidney incompatibility",often combined with phlegm-heat disturbance of the heart,stomach disorders,and liver depression and spleen deficiency.The basic treatment method is to communicate the heart and kidney,combined with the treatment methods of clearing away heat,soothing the liver,and the stomach,while taking care of soothing the nerves.She is good at compatibility and uses medicines cleverly.This article uses data mining to sort out and analyze medical records,and combine clinical manifestations,tongue conditions,pulse conditions,pathogenesis,treatment methods,etc.,to explore the experience of the tutor in the diagnosis and treatment of insomnia from the perspective of "heart-kidney incompatibility".To some degree all the above provides new ideas and methods for the clinical diagnosis and treatment of insomnia.
