Study On The Changes Of TCM Syndrome Types And Syndromes Before And After XELOX Chemotherapy In Advanced Gastric Cancer

Purpose:In this project,a prospective research method was used to analyze the evolution of TCM syndromes and syndromes in patients with advanced gastric cancer before and after XELOX chemotherapy,investigating the effect of XELOX chemotherapy on TCM syndrome types and syndrome changes in advanced gastric cancer,to clarify the law of syndrome types and syndrome changes,so as to carry out targeted early intervention of traditional Chinese medicine,and provide guidance for the diagnosis and treatment of advanced gastric cancer by integrating Chinese and Western medicine.Method:From September 2019 to December 2020,50 patients with advanced gastric cancer who met the clinical criteria were in the wards or outpatients of Jiangsu Provincial Hospital of Traditional Chinese Medicine and Jiangsu Provincial Cancer Hospital.The clinical observation record table was made to collect the four diagnostic data of the patients at the three time points of the day before chemotherapy,the 21 and 42 day after chemotherapy,and the syndrome differentiation was carried out according to the TCM diagnostic criteria of gastric cancer.SPSS26.0 was used to establish the database for statistical analysis.Results:(1)Of the 50 patients with advanced gastric cancer,male patients accounted for a relatively large proportion(78%),39 patients,and female patients accounted for a relatively small proportion(22%).There were 11 patients,61-70 years old as a high-risk population;the pathological staging was based on IVB Mainly,accounting for 68%.(2)Before chemotherapy(at the time of enrollment),the two types of syndromes of deficiency of qi and blood and syndrome of qi stagnation and blood stasis are the most common,accounting for 32%and 22%respectively,and gastric heat injury yin syndrome(4%)The least;TCM syndromes are mostly qi deficiency,blood deficiency,qi stagnation,and blood stasis,accounting for 40%,34%,30%,and 24%,respectively;symptoms include pain and discomfort in the gastric cavity and distension after eating.,Pale complexion,fatigue,etc.The most common tongue quality is pale red tongue(32%)and pale tongue(34%).The most common tongue coating is thin white coating(48%),and the pulse is narrow pulse(30%).Xuan mai(34%)is more common.(3)21 days after chemotherapy(one cycle of chemotherapy),TCM syndromes are mostly qi and blood deficiency syndrome(50%),qi stagnation and blood stasis syndrome(6%)accounted for less than before,stomach heat injury yin syndrome(24%)In terms of syndromes,the proportion of Qi stagnation syndrome(14%)and blood stasis syndrome(10%)is relatively reduced,while the proportion of Yin deficiency syndrome(32%)is significantly higher;the symptoms include fatigue,weight loss,Loss of appetite,loose stools,etc.are in the front,loss of appetite,nausea and vomiting,fatigue,five upset fever,hot flashes,night sweats,spontaneous sweating,loose stools and other symptoms significantly increase,stomach pain and discomfort,post-eating bloating,abdominal distension,abdominal distension,acid reflux Tongue quality is mainly pale white tongue,accounting for 50%,tongue coating is usually thin white coating(54%)and less coating(24%);thin veins(50%)are significantly increased,string pulse(6%)Significantly reduced.(4)On the 42nd day after chemotherapy(two cycles of chemotherapy),the proportion of gastric-heat injury-yin syndrome and Qi-blood deficiency syndrome increased further;the proportion of deficiency syndromes showed an overall upward trend,with Qi deficiency syndrome(72%)and blood deficiency syndrome(72%).60%),Yin-deficiency syndrome(44%)showed an upward trend;stomach pain and discomfort,bloating after eating,irritability,loose stools and other symptoms were relieved compared with the previous one,pale complexion,fatigue,hot flashes and night sweats,five upset fever The tongue coating is still dominated by thin white fur and less fur,which accounts for little change in d21;the pulse condition is dominated by thin pulses(56%),Xuan pulse(6%)is significantly reduced,and the number of pulses is somewhat Increased,the increase rate increased from 6%before chemotherapy to 22%,while the changes of Huamai and Shen Ximai before and after chemotherapy were smaller.Compared with d21 after chemotherapy,there was no statistical significance in the changes of syndromes and syndrome types on d42 after chemotherapy.Conclusions:Before and after XELOX chemotherapy in the treatment of advanced gastric cancer,The patterns of TCM syndrome types and syndromes can be followed regularly.Before chemotherapy,the syndrome types were mostly qi and blood deficiency syndrome and qi stagnation and blood stasis syndrome.After chemotherapy,qi and blood deficiency syndrome and yin deficiency internal heat proved to increase significantly.Before chemotherapy,there are more syndromes such as Qi deficiency,blood deficiency,stagnation of Qi,and blood stasis.After chemotherapy,syndromes such as Qi deficiency,blood deficiency,and Yin deficiency are the main ones.In conclusion,after XELOX treatment of advanced gastric cancer patients,the loss of Qi and blood can be aggravated and the yin fluid of the human body can be damaged at the same time.Therefore,patients undergoing XELOX chemotherapy can be treated with traditional Chinese medicine in advance,and while replenishing qi and blood,they should also pay attention to clearing away heat and nourishing yin.
