Comparison Of Efficacy Between TADA Regimen And Other Chemotherapy Regimens In The Treatment Of Multiple Myeloma

Abstract/Summary:
Objective : To compare TADA regimen(Arsenite,Thalidomide,Dexamethasone,Vitamin C)with other chemotherapy regimen in the treatment of multiple myeloma,and explore the efficacy of arsenite acid in the treatment of myeloma,so as to provide reference for clinical research and selection of treatment regimen.Methods : Retrospective analysis was performed on 40 cases of multiple myeloma patients admitted to Department of Hematology,Affiliated Hospital of Yanbian University from 2005 to 2018.They were divided into two groups according to chemotherapy regimens,20 patients in TADA chemotherapy group,and 20 patients in the other regimens chemotherapy group.Compare the efficacy and side effects of the two groups.At the same time,make a statistical analysis of the gender,age,ECOG score and laboratory indicators in the two groups.The number of MM patients needing blood transfusion and using antibiotics during 3 cycles of chemotherapy in the two groups was statistically analyzed,and the efficacy and adverse reactions after chemotherapy were compared.The t test was used for the comparison between the measurement data groups with normal distribution,the rank sum test was used for the measurement data with non-normal distribution,and the x2 test was used for the counting data.The Kaplan-Meier method was used for survival analysis.P<0.05 was considered statistically significant.Result:Baseline data of the two groups were compared,and gender,age,ECOG score,classification and staging were compared between the two groups,with P<0.05,showing no statistical significance.Comparison of laboratory indicators before and after treatment between the two groups showed that there was no statistical difference in laboratory indicators between the two groups before treatment,and there was no statistical difference in laboratory indicators between the two groups after 3 courses of treatment.After treatment,the levels of β2 microglobulin,the proportion of bone marrow plasma cells,creatinine and erythrocytic sedimentation rate in TADA group were all lower than before treatment(P<0.05),and the differences were statistically significant.The levels of β2 microglobulin,the proportion of bone marrow plasma cells,creatinine and erythrocyte sedimentation in other regimen groups were all lower than before treatment(P < 0.05),and the differences were statistically significant.Comparison of the number of patients needing blood transfusion during the 3courses of chemotherapy showed statistically significant difference between the two groups(P<0.01).The number of patients needing antibiotics in the TADA group was significantly less than that in the other chemotherapy groups,with statistically significant difference(P< 0.05).After 3 cycles of chemotherapy,the total response rate of patients in the TADA group was 40%(8 cases),and the total response rate of 20 patients in the other regiments was 60%(12 cases).Comparison between the two groups showed no statistically significant difference(P>0.05).In the TADA chemotherapy group,there was 1 case achieved complete response(CR)(5%)and 1 case achieved very good partial response(VGPR)(5%),Partial response(PR)6 cases(30%).In the other regiments chemotherapy group,there were complete response(CR)in 2 patients(10%),very good partial response(VGPR)in 1 patient(5%),and partial response(PR)in 9cases(45%).Compared with other chemotherapy regiments,the incidence of hematological toxicity was 15% and 50%,and the difference was statistically significant(P=0.018).The incidence of gastrointestinal reactions was 10% and 40%,respectively,P=0.028,and the difference was statistically significant.The incidence of edema in the two groups was 15% and 5%,respectively,P=0.598,the difference was not statistically significant.The incidence of peripheral neuropathy was 5% and 15%,respectively,with no statistically significant difference(P=0.151).The incidence of liver injury was 10%and 20%,P=0.658,the difference was not statistically significant.The incidence of rash was 0 and 20%,P=0.114,and the difference was not statistically significant.Conclusion:1.The TADA regimen was comparable to other chemotherapy regimens in terms of overall response rates.2.The TADA regimen required fewer transfusions than the other chemotherapy regiments,and the number of people requiring antibiotics was significantly lower than the other chemotherapy regiments.3.Adverse reactions of TADA regimen were significantly lower than those of other chemotherapy regimens.
