Purpose: 1.To compare the difference between the color of dental implants and natural teeth with the same name during the period of gingival shaping in the aesthetic zone,and to explore the regularity of the color of dental implants over time.2.Measure the changes in the crown and root direction of the dental implants during the period of gingival restoration in the aesthetic area,and compare the differences;explore the change rules of each landmark in each time period of shaping.3.Use intraoral scanning combined with reverse engineering software to measure the 2D change in the gingival lip-palatine direction of the dental implant and the 3D change in the three-dimensional coordinate axis,and the 4D change in volume over time during the period of dental implant restoration in the aesthetic zone.The relationship between the amount of time period change.Materials and methods: We selected 20 patients who were treated in the implant center of our hospital from October 2019 to December 2020,who underwent implant restoration in the aesthetic zone and agreed to gingival shaping.Among them,12 were males and 8 were females,aged 19-56 years old,with an average age of(37.7±13.3)years old.Experiment 1: Starting from the day of shaping,taking pictures of the aesthetics area during monthly review,selecting 0M(the day of shaping),3M(3 months of shaping)and 6M(6 months of shaping)at the same angle and field of view Import the photos from Photo Shop software to measure the color value of the gums of the implants and their teeth with the same jaw and the same name(control teeth).Experiment 2: From the day of shaping,the aesthetic area centered on the dental implant was photographed during the monthly review,and the length of the adjacent teeth of the patient was measured with a digital caliper in order to convert the scale on the measurement software.Select 0M,3M and 6M photos and import them into the Photo Shop software.Use the horizontal tangent line passing through the highest point of the free gingiva of the proximal teeth as the reference line to make the vertical gingival papilla and the midpoint of the gingival margin to the reference line.Line,measure the length of the line segment and record the coordinate value of each measurement point,and calculate the change in the crown-root direction of the gingival landmark point during the shaping process.Experiment 3: From the day of shaping,3Shape Trios scanner was used to prepare the digital dentition model of the anterior tooth area during the monthly review.The mouth scan data of 0M,3M and 6M were selected and converted using Exchange2016 software.In the stl format,two sets of two are imported into the reverse engineering software Geomagic Control to measure the 2D change of the labial gums of the implanted tooth in the "lip-palatine direction" and the 3D change "on the three-dimensional axis";in the Geomagic studio software Complete the measurement of the 4D change of "volume change over time".Results: Experiment 1: During the period of gingival shaping with implant restoration in the aesthetic zone,the gingival color L* value of dental implants was lower than that of natural gingiva(P<0.05);the gingival color b* value of dental implants was lower than the b* value of natural gingiva(P<0.05);the a* value of the midpoint of the implant gingival margin is higher than 0M at 3M(P<0.05);the chromaticity between the midpoint of the implant gingival gingival papilla,the midpoint of the free gingival margin and the attachment site The differences in values were statistically significant(P<0.05).Experiment 2: During the period of gingival shaping with implant restoration in the aesthetic zone,the change of the gingiva at 0-3M mesial gingival papilla,free gingival margin midpoint,and distal gingival papilla crown-to-root change is:(0.44±0.37)mm,(0.31±0.21)mm,(0.61±0.49)mm;3-6M changes in order :(0.44±0.37)mm,(0.23±0.21)mm,(0.53±0.37)mm;0-6M changes in order :(0.74±0.46)mm,(0.41±0.40)mm,(0.86±0.47)mm.The 0-6M changes of the proximal and distal gingival papillae were greater than the midpoint of the gingival margin(P<0.05).The 0-6M change of mesial gingival papilla shaping was greater than 0-3M and 3-6M(P<0.05).Experiment 3: The amount of gingival changes during gingival shaping with implant restoration in the aesthetic zone varies from positive to negative at different sites,and the values are different.The parts with regular gingival changes were mainly concentrated within 2mm of the root of the neck margin.From the perspective of 2D and 3D levels,the average positive change of 0-3M is greater than 3-6M(P<0.05);the average positive change of the whole six months of shaping is greater than 3-6M(P<0.05),but it is less than 0 There was no statistically significant difference in the positive change of-3M(P>0.05).The average 4D change of gingival volume: 0-3M:(11.19±10.73)mm~3;3-6M:(1.57±9.64)mm~3;0-6M(8.10±13.89)mm~3.Conclusion: 1.Dental implants and natural teeth with the same name and the same name have significant differences in gingival color;implant gingival color changes during the shaping process,and there are significant differences in the color of different sites of the gingiva.2.During the shaping process of the lip-side gingiva of the dental implant in the aesthetic zone,the gingival papilla is continuously filled and the gingival margin curve is gradually coordinated;the crown-root change of the gingival papilla is significantly greater than the free gingival margin midpoint.3.The gums of dental implants in the aesthetic zone showed an overall increasing trend during the shaping period.The gingival changes from 2D to 4D changes on the lip and palate side all show that the 0-3M change is greater than 3-6M.Among them,the gingival changes of 2mm near the root of the cervical margin showed a certain regularity,and the changes were mostly positive.
