Objective:To compare the effects of different endometrial preparation schemes on the outcome of frozen embryo transfer in patients with endometriosis(EMS)and infertility,and to discuss the application value of gonadotropin releasing hormone agonist(GnRH-a)combined with hormone replace treatment in patients with endometriosis and infertility.Materials and methods:The data were collected from the Second Affiliated Hospital of Jilin University from December 2020 to December 2020.The cycle data of endometriosis related infertility patients who underwent frozen embryo transfer(FET)were retrospectively analyzed.According to the endometrial preparation protocol,they were divided into natural cycle group(n = 32),hormone replace treatment group(HRT,n = 32),GnRHa + HRT(n = 30).The clinical data(age,years of infertility,body mass index,type of infertility,ovarian endometriosis cyst,basal hormone level,number of transferred embryos and the thickness of endometrium on the embryo transfer day,embryo recovery rate,high-quality embryo rate)were collected,and the differences of serum indexes and pregnancy outcome indexes(embryo implantation rate,clinical pregnancy rate,early abortion rate and ectopic pregnancy rate)on the day of embryo transfer between different endometrial preparation schemes were compared.There are differences.Results:There were no significant differences in patients’ age,years of infertility,BMI,basal hormone levels of FSH,LH,E2,P,T,PRL,number of embryo cycles,endometrial thickness on the day of transplantation,embryo recovery rate and high-quality embryo rate)among patients with different endometrial preparation schemes.There was significant difference in serum E2 level between different endometrial preparation protocols on the day of embryo transfer(P < 0.05).Natural cycle group was significantly lower than artificial cycle group(P < 0.05).It was also significantly lower than that in GnRH-a + HRT group(P < 0.05).There was no significant difference between hormone replace treatment group and GnRH-a + HRT group(P > 0.05).No significant difference was found in serum P on the day of embryo transfer.Compared with the other two control groups,the serum CA125 level of GnRH-a + HRT group on the day of embryo transfer was obviously lower,and the difference was significant(P<0.05).The rate of implantation and pregnancy in different subgroups showed significant differences(P = 0.01 and 0.016,respectively).The clinical pregnancy rate was significantly high in GnRH-a + HRT group,compared with others.The rate of early abortion and ectopic pregnancy showed no significant difference among different endometrial preparation schemes.Conclusion:Compared with natural cycle group and hormone replace treatment group,GnRHa + HRT regimen can improve the cycle transfer outcome of patients with endometriosis and infertility,improve the embryo implantation rate and clinical pregnancy rate of FET cycle,and reduce the serum CA125 level.
