Abstract/Summary:
Objective:To explore the value of three-dimensional reconstruction in preoperative evaluation of pancreatoduodenectomy.Methods:Collected in January 2019 to October 2020,50 patients with pancreatic and periampullary occupation were diagnosed in the first Hospital of Jilin University,of these,29 were male,with an average age(60.34±8.25)years and 21 were female,with an average age(58.62±11.63)years.All patients underwent 256 rows of abdominal enhancement CT scans to obtain 1 mm of thin-layer imaging data.To the enhanced CT imaging data in DICOM format with the 3D visualization system of abdominal medical images.Analysis of enhanced CT data and three-dimensional reconstruction data to determine the location,morphology,relationship with peripheral blood vessels,tissues,organs and vascular variation,to complete the preoperative resectability,and then compared with the actual situation in the operation and postoperative pathology.Results:Of the 50 patients,35 were pancreatic cancer,12 cases of terminal cholangiocarcinoma,Vater ampullary carcinoma 2 cases,A case of intraductal papillary mucoma(IPMN)of pancreas;Hepatobiliary,pancreatic and splenic enhancement CT judging tumor resectable 29 cases,21 cases of unresectable patients;Three-dimensional reconstruction of 31 patients with resectable tumors,in 19 unresectable patients.Compared with the enhanced CT.,the sensitivity,specificity and accuracy of 3D reconstruction were higher And postoperative pathology was R0 resection,preoperative 3D reconstruction evaluation has extremely high credibility.Conclusion:3D reconstruction technology is of great help to the preoperative evaluation of pancreatoduodenectomy,and its sensitivity and specificity are higher than that of enhancement CT;Through preoperative 3D reconstruction,tumor resectability can be more accurately evaluated,and whether there is any variation in blood vessels.3d reconstruction technique can be used as a new method for preoperative evaluation of pancreatoduodenectomy to achieve precision and personalized medicine.
