Objective:Objective to investigate the correlation between serum CRP/ALB ratio and disease activity of ulcerative colitis.Method:241 patients with ulcerative colitis who were diagnosed and treated in our hospital from October 2018 to December 2020 were analyzed retrospectively.According to the consensus opinions on diagnosis and treatment of inflammatory bowel disease(Beijing,2018),241 UC patients were divided into 56 patients in remission period and 185 in active period according to the severity of the disease 44 cases were mild,64 were moderate active and 77 were severe.The differences of serum CRP/ALB ratio,CRP,ALB,ESR,FIB,PLT,Hb,RDW,NLR,PLR and LMR were compared between patients with UC in remission and activity and different severity,and the correlation between crp/alb and other laboratory indicators and UC disease activity was further analyzed.Statistical methods:Chi square test,rank sum test,Spearman correlation analysis,multivariate logistic regression analysis,and the diagnostic value of these indexes was determined by ROC curve and calculation of AUC value by using Jordan index.P<0.05 was statistically significant.Result:(1)CRP/ALB ratio,CRP,ESR,PLT,FIB,NLR and PLR of UC patients in activestage were significantly higher than those in remission stage(P<0.01),while ALB and LMR of UC patients in active stage were significantly lower than those in remission stage(P<0.01).(2)CRP/ALB ratio,CRP,ESR,FIB,NLR and PLR of UC patients in severe active stage were significantly higher than those in mild and moderate active stage(P<0.01),while ALB and LMR of UC patients in severe active stage were significantly lower than those in mild and moderate active stage(P<0.01);ESR and PLT of UC patients in moderate active stage were significantly higher than those in mild active stage(P<0.05),However,there was no significant difference in serum CRP/ALB ratio,CRP,ALB,FIB,NLR,PLR and LMR levels between mild and moderate active UC patients(P>0.05)(3)CRP/ALB ratio,CRP,ESR,FIB,PLT,NLR and PLR were positively correlated with UC activity(P<0.01),ALB and LMR were negatively correlated with UC activity(P<0.01),and the absolute r_s value of CRP/ALB ratio was higher than other serological indexes;CRP/ALB ratio,CRP,ESR,FIB,PLT,NLR and PLR were positively correlated with UC disease severity(P<0.01),ALB and LMR were negatively correlated with the severity of UC active disease(P<0.01),and the absolute r_s value of CRP/ALB ratio was higher than other serological indexes.(4)Multivariate logistic regression analysis showed that CRP/ALB ratio,PLT and NLR were independent risk factors of UC disease activity,and the odds ratio of CRP/ALB ratio in evaluating UC disease activity was 3.484(95%confidence interval1.008-12.040),which was better than PLT and NLR;when predicting severe UC in active phase,only CRP/ALB ratio was independent risk factor of severe UC in active phase,and the odds ratio was 6.157(95%confidence interval 3.005-12.614).(5)The diagnostic value of UC clinical activity was evaluated by ROC curve.When the CRP/ALB ratio threshold value was 0.079,AUC was 0.901,sensitivity and specificity were77.8%and 87.5%respectively,which was better than that of CRP,ALB and other indicators alone.(6)ROC curve was used to evaluate the diagnostic value of each index in predicting severe UC in active UC.When the optimal critical value of CRP/ALB ratio was 0.646,the AUC value for identifying severe UC was 0.877,the sensitivity and specificity were 79.2%and 84.3%respectively,which were better than CRP,ALB and other indexes alone.Conclusion:1.serum CRP/ALB ratio was positively correlated with UC disease activity,which was an independent risk factor for UC disease activity.2.serum CRP/ALB ratio is an independent risk factor for severe UC in active stage,which can be used as a new index to predict severe UC.3.serum CRP/ALB ratio was better than CRP,ALB,ESR,FIB and CBC in evaluating UC disease activity and predicting severe UC.
