| Objective:Compare the effectiveness and safety of single-use digital flexible ureteroscope and multimodular Flexible ureteroscope in the treatment of upper urinary calculi.Method:The study selected 78 patients with upper urinary calculi diagnosed and treated in our hospital from October 2019 to December 2020 for retrospective analysis.According to the type of flexible ureteroscope,48patients in SD-FURS group used Zebra TMsingle-use digital flexible ureteroscope combined with holmium laser to treat upper urinary calculi,30 patients in M-FURS group used Polyscope multimodular Flexible Ureteroscope combined with holmium laser to treat upper urinary calculi.Statistical comparative analysis of the age,gender,Body Mass Index,stone size,stone location,stone CT value,state of Hydronephrosis,operation time,stone free rate,laboratory index changes before and after operation,and postoperative complications of the two groups were statistically compared and analyzed.And other indicators.Result:The 78 patients with upper urinary calculi enrolled in the group were all successfully completedGeneral data comparison:age,gender,Body Mass Index,stone size,stone location,stone CT value,state of hydronephrosis and other preoperative indicators,the difference between the two groups was not statistically significant(P>0.05)Comparison of operation time:The average operation time in the SD-FURS group was 52.56±11.41 min,and the average operation time in the M-FURS group was 70.13±16.48 min.The difference between the two groups was statistically significant(P<0.05).Patients were distinguished according to different stone positions,and the operation time was compared between the two groups.Calculi in the lower kidney(SD-FURS group 61.36±13.25min,83.00±15.88min in the M-FURS group);renal pelvic calculi(52.27±10.04min in the SD-FURS group,68.75±14.93min in the M-FURS group);calculi in the upper ureter(SD-The FURS group was 48.00±9.38 min,and the M-FURS group was 59.64±10.44 min).In patients with different stone positions,the difference in operation time between the groups was statistically significant(P<0.05).Comparison of laboratory indicators:Comparison of changes in white blood cell,hemoglobin,and Serum creatinine between SD-FURS group and M-FURS group before and after surgery,the differences were not statistically significant(P>0.05).Comparison of stone free rate:The total stone free rate(SFR)of SD-FURS patients was 87.50%,and the total SFR of M-FURS patients was66.67%.The difference in total SFR between the two groups was statistically significant(P<0.05).Separate statistical analysis according to different positions of the Calculi,in patients with calculi in the lower kidney(90.90%in the SD-FURS group,40.00%in the M-FURS group),the difference in SFR between the two groups was statistically significant(P<0.05),Patients with renal pelvic and upper ureteral calculi,there was no statistically significant difference in SFR between the two groups(P>0.05).Comparison of postoperative complications:In the SD-FURS group,there were 48 patients with postoperative fever,6 patients with urinary sepsis,9 patients with hematuria,and 11 patients with back abdominal pain.There were 30 patients in the M-FURS group,4 had fever,6 had hematuria,and 9 had back abdominal pain.The two groups were subjected to statistical analysis of various complications,and there was no significant difference between the groups(P>0.05).Conclusion:1.Compared with the multimodular Flexible Ureteroscope,the single-use digital flexible ureteroscope has better controllability and is easy to learn.2.Single-use digital flexible ureteroscope and multimodular flexible ureteroscope are safe and effective in the treatment of upper urinary calculi≤2cm.3.Compared with the multimodular flexible ureteroscope,the single-use digital flexible ureteroscope can shorten the operation time and improve the stone free rate,especially in the treatment of lower calyx calculi. |
