Objective:To compare the safety and effectiveness of negative pressure combined ureteroscope combined with flexible ureteroscope in the treatment of renal calculi and flexible ureteroscope in the treatment of renal calculi,and to further explore the advantages and disadvantage,s of the two treatments,so as to provide an evidence for clinical decision-making.Methods:A retrospective analysis was made on 42 cases of renal calculi in our hospital from March 2017 to October 2018.They were divided into two groups:negative pressure combined ureteroscope combined with flexible ureteroscope group(combined group)and flexible ureteroscope group(flexible ureteroscope group).Negative pressure combined ureteroscope combined with flexible ureteroscope and flexible ureteroscope were used to treat renal calculi.Among them,18 cases were in combined group and 24 cases were in flexible ureteroscope group.The perioperative data of these patients were collected.T test,rank sum test and chi-square test were used to analyze the data.By comparing the age,sex,body mass index(BMI),calculi location,calculi size,left and right sides of the calculi and preoperative biochemical indicators,statistical analysis showed that there was no significant difference between the two groups before operation.By comparing the operation time,intraoperative ureteral injury rate,stone free rate 1 day and 1 month after operation,hospital stay after operation,biochemical indicators before and after operation,the safety and effectiveness of the two treatments were analyzed.Results:Compared with the flexible ureteroscope group,the operation time,hospitalization time,white blood cell count,neutrophil ratio,stone free rate on the first day after operation,procalcitonin,urine white cell count,cost and incidence of complications are significantly different in the combined group(P<0.05).There was no difference in intraoperative ureteral injury,stone free rate in 1 month after operation,success rate of intraoperative endoscopic sheath insertion and creatinine after operation(P>0.05).There were differences in white blood cell count,neutrophil ratio before and after operation in the two groups respectively(P<0.05).There was no difference in creatinine before and after operation in the two groups(P>0.05).There were significant differences in procalcitonin and urine white cell count before and after operation in flexible ureteroscope group(P<0.05),while there was no significant difference in procalcitonin and urine white cell count before and after operation in combined group(P>0.05).Conclusion:The combined group has lower burden of removing calculi,lower incidence of complications,short hospital stay and lower infection rate.There is no difference in the final stone free rate between the two groups,and there is no significant difference in the success rate of endoscopic sheath insertion,renal function and ureteral injury between the two groups.In addition,the combined group has longer operation time and more cost.
