Significance Of Detecting Neonatal Cord Blood PCT,IL-6 And CRP On Early Premature Rupture Of Membranes

Objective: to explore the significance of detecting PCT,IL-6 and CRP in neonatal cord blood on premature rupture of membranes and neonatal early sepsis(EONS).Methods: A total of 90 fetuses who came to our hospital for full-term delivery from September 2019 to September 2020 were selected.Among them,9 full-term babies with EONs due to premature rupture of membranes were the study group,and 81 normal full-term babies were the control group.the umbilical cord blood of the two groups of newborns were drawn after birth,and the levels of PCT,IL-6,and CRP in the umbilical vein blood of the two groups were recorded.The differences between the two groups were compared,and ROC curves were drawn according to these three inflammatory indicators.Find the best combination.Results: Comparing the levels of PCT,IL-6 and CRP in the cord blood of the study group and the control group,the difference was found to be significant(P<0.05);the ROC curve was made to obtain the three inflammatory indicators of cord blood in this study to diagnose the cutoff of EONS The values correspond to6.49ng/ml,73.11pg/ml,1.52mg/L,and the AUC are PCT: 0.778,IL-6: 0.764,CRP: 0.821.CRP combined with IL-6 draws the ROC curve to get the area under the curve to be 0.868,PCT combined with CRP draws the ROC curve to get the area under the curve to get0.830,PCT combined with IL-6 draws the ROC curve to get the area under the curve to be0.831,the sensitivity,specificity,positive predictive value,and negative predictive value of CRP combined with IL-6 are 66.7%,96.3%,94.74%,and 74.3%,and the sensitivity,specificity,positive predictive value,and negative of PCT combined with CRP The predictive values are all 88.9%,72.8%,75.78%,86.57%.The sensitivity,specificity,positive predictive value,and negative predictive value of PCT combined with IL-6 are all77.8%,90.1%,88.71%,80.23%,PCT,IL-6,CRP draw the ROC curve,and the area under the curve is 0.909.When the three are combined,the sensitivity,specificity,positive predictive value,and negative predictive value are all 88.9%,96.3%,96%,and 89.67%.Conclusion: The three indicators of neonatal cord blood detection alone have guiding significance for the diagnosis of early-onset neonatal sepsis,and the combined detection AUC is higher than the single index detection,which can guide the clinic to a certain extent.
