Objectives: To evaluate and compare the performance of the early-onset sepsis(EOS)risk calculator and some blood biomarkers in predicting neonatal EOS.Methods: This was a retrospective case-control study of neonates who were ?34 weeks of gestation and ?12 hours of age at admission of our children's hospital during 2016-2018.Neonates with strictly defined EOS(infected group,n=353)and without evidence of infection(uninfected group,n=148)was included in this study.We reviewed the charts of the501 neonates and analyzed their data.The risk and management categories for all neonates were calculated using the EOS calculator,and these results were analyzed and compared with laboratory data to assess the predictive values.Results: Comparing all the most frequently used predictors in clinics,procalcitonin(PCT)had the best predictive value(sensitivity: 87.5%,specificity: 95.5%,AUC: 0.929,P<0.05),closely followed by EOS calculator(sensitivity: 81.2%,specificity: 93.9%,AUC 0.910,P<0.05).The combination of EOS calculator,complete blood count(CBC),C-reactive protein(CRP)and PCT further improved the predictive efficiency(sensitivity: 97.2%,specificity: 93.2%,AUC 0.987,P<0.05).Conclusions: EOS calculator and PCT showed good predictive values comparing to conventional test of CBC and CRP.
