ObjectiveTo observe the clinical efficacy of remote ischemic post-conditioning(RIPost C)on patients with acute cerebral infarction,and the serum concentrations changes of hypoxia inducible factor-1α(Hypoxia inducible factor-1α,HIF-1α)and survivin(Survivin).To explore the clinical efficacy and possible mechanism of RIPost C on patients.MethodsSelected patients with acute cerebral infarction who were treated in the Department of Neurology of Shenyang Medical College Affiliated Central Hospital from March 2019 to January 2021.The onset time of the patients was within 24 hours,and 100 patients with NIHSS score of 3-16 were selected according to randomization Number table method,50 people in the experimental group and 50 people in the control group.There was no difference in age and gender between the two groups(P>0.05).The control group was given conventional treatment: double anti-platelet aggregation,lipid-lowering and stabilizing plaque,promoting blood circulation and removing blood stasis,removing free radicals,and nutrient nerve treatment.The experimental group was treated with RIPost C at the same time as the conventional treatment.On the 1st and 10 th day,the patient’s NIHSS score was recorded,the patient’s elbow blood was drawn,centrifuged in the laboratory,and the serum drawn was stored in the-80℃refrigerator.The patient’s NIHSS score is used as an indicator of the degree of recovery,and the effective rate of the two groups of patients can be compared through the changes in the NIHSS score.The concentration changes of HIF-1α and Survivin in serum samples of the two groups of patients can be detected by ELISA kit,and SPSS25.0software was used for statistical analysis.Results1.The effective rate of the experimental group was 88%,and that of the control group was 62%.The effective rate of the experimental group was higher than that of the control group,and the difference was statistically significant(P<0.05).2.Comparison of the recovery of neurological function deficit: intra-group comparison showed that the NIHSS score of the experimental group after treatment was lower than that before treatment,while the NIHSS score of the control group after treatment was lower than that before treatment,with statistical significance(P<0.05).Comparison between groups,the NIHSS score after treatment of the experimental group was lower than the NIHSS score after treatment of the control group,and the difference was statistically significant(P<0.05).3.Comparison of HIF-1α : intra-group comparison showed that the concentration of HIF-1α in the experimental group after treatment was lower than before treatment,while that in the control group was lower than before treatment,with statistical significance(P<0.05).Comparison between groups,the concentration of HIF-1α after treatment in the experimental group was lower than the concentration of HIF-1α after treatment in the control group,and the difference was statistically significant(P<0.05).4.Comparison of Survivin: intra-group comparison showed that Survivin concentration after treatment was higher than that before treatment in the experimental group.Survivin concentration after treatment was higher in the control group than before treatment,with statistical significance(P<0.05).Comparison between groups,survivin concentration after treatment in the experimental group was higher than survivin concentration after treatment in the control group,with statistical significance(P<0.05).Conclusion1.Both groups are effective in treating acute cerebral infarction.RIPost C plus conventional treatment is more effective,which can better reduce the NIHSS score and improve neurological deficits.2.RIPost C reduces the concentration of HIF-1α in the patient’s serum,because HIF-1α is positively correlated with the degree of hypoxia,so it is speculated that RIPost C can improve the degree of cerebral hypoxia in patients.3.RIPost C can increase the serum concentration of Survivin in patients.Therefore,it is speculated that RIPost C may inhibit nerve cell apoptosis,promote new blood vessels,and improve the symptoms of neurological deficits.
