Clinical Study Of Huatan Tongluo Decoction In Treating Acute Ischemic Stroke Caused By Cerebral Small Vascular Disease With Wind-Stagnation Syndrome

ObjectiveThis study mainly explored the clinical efficacy and safety of Huatan Tongluo Decoction in patients with acute ischemic stroke caused by cerebral small vessel disease,as well as its improvement in neurological deficits and syndromes of traditional Chinese medicine.In the hope of using the method of TCM syndrome differentiation and treatment,to provide individualized TCM treatment plan for patients with acute ischemic stroke caused by Cerebral Small Vascular Disease with Wind-Stagnation Syndrome.MethodA selection of 42 patients with acute ischemic stroke caused by Cerebral Small Vascular Disease with Wind-Stagnation Syndrome who were admitted to the Department of Neurology of Wuhan NO.1Hospital from December 2018 to December 2020 and met the inclusion criteria were divided into observations using a random number table.Group and control group,21 cases in each group.The control group was given clopidogrel 75 mg once in the morning + atorvastatin calcium tablets 20 mg once every night;the observation group was given clopidogrel 75 mg once in the morning + atorvastatin calcium tablets 20 mg once every night + experience prescription Huatan Tongluo Decoction is flavored,one dose per day,200 ml each time,twice in the morning and evening after meals,taking it for 21 consecutive days.Huatan Tongluo Decoction was given to patients in the form of decoction of traditional Chinese medicine in the hospital of Wuhan No.1 Hospital.At the same time,both groups were given conventional treatments such as nerve nutrition and circulation improvement.After 21 days of treatment,the two groups of patients were treated with the National Institutes of Health Stroke Scale(NIHSS),Modified Rankin Scale(m RS)and other scales and TCM syndrome score evaluation,and analyzed the correlation by self-control before and after control and between groups.For the improvement of indicators,symptoms,and conditions,routine safety monitoring of patients during the treatment process,such as liver and kidney function,coagulation function,electrocardiogram,etc.,and record adverse reactions.Use SPSS 25 statistical software for data analysis.Comprehensive evaluation of the efficacy and safety of Huatan Tongluo Decoction in the treatment of acute ischemic stroke caused by Cerebral Small Vascular Disease with Wind-Stagnation Syndrome.Result1.Before treatment,there was no significant difference between the two groups of patients in gender,age,body mass index,blood pressure,BMI value,previous basic medical history,adverse habits,NIHSS score,m RS score,TCM syndrome score and other baseline data(P>0.05),the results showed the data is comparable.2.Comparison of NIHSS and m RS scores within and between the two groups before and after treatment: the observation group after treatment compared with before treatment,there was a significant difference(P<0.01);the control group before treatment compared with the control after treatment,the score was significantly different(P <0.01);There was no statistical difference between the two groups after treatment(P>0.05).The total effective rate of neurological deficits in the observation group was 95%;the total effective rate of neurological deficits in the control group was81%.The results show that both groups can improve the symptoms of neurological deficit to a certain extent.3.Comparison of TCM syndrome scores: After treatment in the observation group,the scores of TCM syndromes decreased significantly,and the difference was statistically significant(P<0.05);the observation group was compared with the control group after treatment,the former TCM syndromes The scores decreased significantly,and the difference was statistically significant(P<0.05);while the TCM syndrome scores of the control group after treatment did not decrease significantly compared to before treatment,and the difference was not statistically significant(P>0.05).The total effective rate of TCM syndrome score improvement in the observation group was 81.0%;the total effective rate in the control group was 23.5%;there was a statistically significant difference in the total effective rate of TCM syndromes between the two groups(P<0.05).The results showed that the improvement of TCM syndromes in the observation group was more obvious than that in the control group,and the curative effect was better.4.Safety analysis: A total of 4 patients had mild side effects and no serious side effects.In the observation group,2 patients developed mild discomfort after taking the medicine,1 case had nausea and vomiting,and 1 case had diarrhea.In the control group,2 patients developed mild discomfort,1 case of loss of appetite and 1 case of constipation were found.Symptoms improved after symptomatic treatment.The number of adverse reactions between the two groups was not statistically significant(P>0.05).ConclusionThe clinical comparison of Huatan Tongluo Decoction combined with western medicine treatment and western medicine alone on acute ischemic stroke caused by cerebral small vessel disease.Observation of its clinical efficacy and safety for patients with Wind-Stagnation Syndrome showed that Huatan Tongluo Decoction was significantly better than the control group in improving TCM syndromes of patients with Wind-Stagnation Syndrome.Compared with western medicine alone,Huatan Tongluo Decoction combined with Western medicine is more effective in improving the symptoms of neurological deficits in patients,and the benefits may be greater.Huatan Tongluo Decoction did not find any serious adverse reactions and toxic side effects during this clinical trial,and its safety is relatively high.Through this study,it is found that Huatan Tongluo Decoction is effective and safe in clinical use,and can be further studied and promoted and applied in combination with clinical practice.
