| ObjectiveTo explore the influence factors of the communication between nurses and patients in the perioperative period of gastrointestinal surgery and Construct a nurse-patient communication scheme based on CICARE Model.To evaluate the influence of this scheme on nurses’ clinical communication ability and the satisfaction from patients.Methods1.Objective sampling method was used to select 14 nurses working in gastrointestinal surgery in Zhou Kou Central Hospital.The semi-structured interview method was used to collect the feelings in the communication between nurses and patients,and the collected data were analyzed by qualitative research content analysis method.2.Based on the previous qualitative research,combine the nurses’ work experience and the communication scheme of nurse-patient in perioperative period,it was initially proposed.On this basis,the perioperative nurse-patient communication scheme of gastrointestinal surgery based on CICARE Model was formed by expert meeting method.3.One hundred nurses were selected from the gastrointestinal surgery work in Zhou Kou Central Hospital,who met the inclusion criteria,and their numbers were simply numbered.According to the method of random number table,50 nurses were selected as the control group and 50 nurses were selected as the experimental.Conventional training methods and routine communication methods were used in the control group,and the practice of CICARE communication mode training and the perioperative nurse-patient communication scheme were added to the experimental group on the basis of the control group.At the end of the trial,50 patients were randomly selected in the control group and the experimental group according to the order of transfer or discharge.Before and after intervention,two groups of nurses were conducted to make theoretical tests and used Clinical Communication Ability and Assessment Scale of Nurses to evaluate themselves.After intervention,the two groups of patients were investigated using Newcastle Satisfaction with Nursing Scales.4.The data were analyzed by the statistical software SPSS 20.0.Statistical methods included descriptive analysis,Wilcoxon rank sum test,t-test,chi-square test,repetitive measures ANOVA and so on.Results1.A total of four themes for 14 nurses were summarized,and they were communication environment,communication consciousness,communication ability and communication literacy.2.Based on the previous qualitative research,the preliminary draft of the program was formed;through the revision of the structure and content of the program by the expert meeting held by the relevant experts of gastrointestinal surgery,nursing management,interpersonal communication,clinical nursing specialty,the perioperative nurse-patient communication scheme based on CICARE Model was formed,which mainly includes knowledge training,communication environment,communication scene,communication process 4 aspects.3.The results showed that after applying the perioperative nurse-patient communication scheme based on CICARE Model,the clinical communication ability of s nurses was significantly higher than that in the control group,and the total mean score of satisfaction and each dimension of the experimental patients were significantly higher than those in the control group.Conclusion1.The factors that affect the communication between nurses and patients in the perioperative period of gastrointestinal surgery include poor communication environment,lack of communication awareness,insufficient communication ability and lack of communication literacy.2.The program constructed in this study transforms the routine nurse-patient communication content into standardized,flow-based nurse-patient communication program,which is operable and reproducible;in addition,the implementation of the program is carried on and integrated into the daily work of nurses.And it is easier for nurses to accept and implement.3.The perioperative nurse-patient communication scheme of gastrointestinal surgery based on CICARE Model can effectively improve the effectiveness of the nurse-patient communication process and greatly improve the satisfaction from the patients,which is worth promoting in clinical application!... |
