The Preliminary Development And Clinical Verification Of The Positive Index Score Scale Of “Heart Arthralgia Syndrome”

Objective: To complete the preliminary development,evaluation and clinical verification of the positive index score scale of“heart arthralgia syndrome”with the characteristics of traditional Chinese medicine,and it is expected that the scale can be used as guiding the Pick complex therapy of“heart arthralgia syndrome”and as a tool for evaluating the curative effect after the therapy of“heart arthralgia syndrome”,which can be widely used in clinic.Methods:1.Preliminary scale preparation:Selecting 276 patients who met the criteria of“heart arthralgia syndrome”(coronary heart disease with numbness of neck,shoulder,waist and leg),the author selected 24 items by means of multiple methods,such as Kronbach coefficient method,retune reliability method,subjective evaluation method,discrete trend method,correlation coefficient method,factor analysis method,and evaluated the reliability,validity and reflection of the scale,and finally completed the preliminary formulation of the positive index score scale of“heart arthralgia syndrome”.2.Clinical verification:Selecting 41 patients with“heart arthralgia syndrome”.Before the patients received the Pick complex therapy treatment,fill in the positive index score scale of“heart arthralgia syndrome”and angina pectoris grading scoring method table;Patients were treated within 12～24 hours and at 4 weeks,they underwent the following two examinations:filling in the positive index score scale of“heart arthralgia syndrome”,angina pectoris grading scoring method,blood routine,urine routine,liver function,kidney function and four coagulation(prothrombin time,ratio of international standard,activated partial thrombin time,thrombin time,fibrinogen concentration).SPSS23.0 statistical software was used for statistical processing and analysis.Results:1.The positive index score scale of“heart arthralgia syndrome”item screening results: There were no exfoliating cases in this study,and 276 cases were eventually observed.18 items were selected as the positive index score scale of“heart arthralgia syndrome”,which is divided into three common factor:patients self checking,medical inspection,medical pulse feeling and palpation.2.Assessment results of the initial positive index score scale of“heart arthralgia syndrome” : the average answer time of the scale is7.47±3.59,less than 20 minutes.The alpha coefficient of kronbach in the initial table is 0.711,the retest reliability is 0.897,the KMO test result is 0.844,the Bartlett test result is 2502.300,(P <0.01).After the maximum variance rotation analysis,the cumulative variance contribution rate was66.605%.3.The preliminary establishment of the positive index score scale of“ heart arthralgia syndrome”for clinical verification.1 case was exfoliated,and 40 cases were observed.After treatment in patients with total score is lower than before treatment in patients with total score,every items after treatment was measured when the measured average decreased to some extent(P <0.01).After three measurements,the correlation between the positive index score scale of“heart arthralgia syndrome”and the angina pectoris grading scoring method table decreased gradually.The last two measurement results of this study indicated that there was a significant correlation between the positive index score scale of“heart arthralgia syndrome”and thrombin time(P <0.05),while the physical and chemical examination showed no significant changes in the blood routine,urine routine,liver function,kidney function and four coagulation(prothrombin time,ratio of international standard,activated partial thrombin time,thrombin time,fibrinogen concentration)(P >0.05).Conclusion:The positive index score scale of “heart arthralgia syndrome”has good reliability,validity and response degree,which has better clinical operability.The scale plays an important role in guiding the treatment of the Pick complex therapy and can be widely used in clinic as a preliminary evaluation tool of the Pick complex therapy.When the Pick complex therapy is used to treat“heart arthralgia syndrome”,it has effected on the liver,kidney and other viscera functions of patients was small,and the safety was high.
