Objective:To explore the clinical efficacy of manual reduction combined with traditional Chinese medicine in the treatment of benign paroxysmal positional vertigo through a randomized controlled clinical study;meanwhile,for the BPPV of vertical semicircular canal with poor effect of conventional manual reduction,to give self-made modified Epley manual reduction,and to count its effective rate.Methods:120 patients were selected and divided into treatment group and control group(60 cases each).The treatment group was treated with manual reduction combined with traditional Chinese medicine,while the control group was treated with manual reduction for 4 weeks.According to the patients’ positional vertigo and positional nystagmus,the statistics were made before the treatment,half an hour after the completion of the initial treatment,one week after the completion of the initial treatment and four weeks after the completion of the initial treatment respectively.The auxiliary evaluation indexes(vertigo degree classification table,dizziness degree classification table,vertigo obstacle scale)were statistically analyzed before the treatment and four weeks after the completion of the initial treatment.BPPV of vertical semicircular canal,which was not effective in conventional manual reduction,was treated with modified Epley manual reduction and its effective rate was counted.The statistical data were processed and analyzed by SPSS 22.0.Results:There was no significant difference between the two groups(P>0.05).Both the treatment group and the control group can have therapeutic effect on BPPV.There were significant differences between the treatment group and the control group in the effective rate,the degree of vertigo and the score of vertigo disorder scale(P<0.05).The effect of the treatment group was better than that of the control group.The effective rate of self-made modified epely maneuver reduction in the treatment of BPPV of vertical semicircular canal with poor effect of conventional maneuver reduction was 71.4%.Conclusion:Manual reduction and manual reduction combined with traditional Chinese medicine have clinical effect in the treatment of BPPV.In improving the vertigo(dizziness)degree,vertigo function and body disorders,manual reduction combined with traditional Chinese medicine is more effective.The BPPV of the vertical semicircular canal,which was not effective in the conventional manual reduction,was affected by the self-made modified epely manual reduction.We believe that manual reduction combined with traditional Chinese medicine is the best way to solve BPPV,which is worth further study and widely used in clinical.
