| Objective:The patients with benign paroxysmal positional vertigo were treated with BPPV and manipulative reset respectively.By analyzing the effective rate of treatment,the improvement of symptoms after treatment and other indicators,the clinical therapeutic effects of the two treatment methods were compared,to evaluate whether BPPV is superior to manual reduction in the treatment of benign paroxysmal positional vertigo.Methods:1.Cases collected:select 60 patients with benign paroxysmal positional vertigo were received treatment in the department of otolaryngology-head and neck surgery,the first affiliated hospital of hebei northern university during April 2019-December 2019.All the selected patients met the diagnostic criteria of BPPV issued by the otolaryngology,head and neck surgery branch of the Chinese medical association in 2017.The patients were between 20 and 80 years old,with no less than 1 week of onset and no other serious diseases or eye and ear diseases.According to the time of first visit,60 patients were randomly divided into the manipulative reduction group and the machine reduction group,including30 patients in the manipulative reduction group and 30 patients in the machine reduction group.2.Treatment plan and method:Each patient filled out the DHI scale before treatment to understand the severity of symptoms.In the manipulative reset group,patients with posterior semicircular BPPV were treated with Epley reduction method;patients with horizontal semicircular BPPV were treated with Barbecue rolloff method;all patients in the machine reset group were treated with BPPV diagnostic apparatus for otostone reduction.After the reduction treatment,the patients sat still for 30minutes,and then the dix-hallpike displacement test and Supine roll displacement test were used to judge the therapeutic effect of the patients.If the effect of the first reduction treatment is invalid,the second reduction can be performed with the same treatment method within 48h after the first reduction,and the effect of the second reduction is the final treatment effect.All patients returned to the clinic 1 day,1 week,1 month and 3 months after treatment,and the therapeutic effects of the two groups were evaluated by DHI scale content and displacement experiment.3.Evaluation of therapeutic efficiency:Through the treatment effect of the two groups of patients,the effective rate of the two reduction methods was compared.The subjective perception improvement of the two reduction methods was compared with the reduced DHI scale scores at 1day,1 week,1 month and 3 months after treatment compared with the DHI scale scores before treatment.By observing whether the specific nystagmus disappeared at 1 day,1 week,1 month and 3 months after the treatment,the objective conditions of the two reduction methods were compared.4.Data processing:SPSS 26.0 statistical software was used for all data analysis,in whichχ~2 test was used for counting data,such as effective rate comparison between machine reduction group and manipulative reduction group.The measurement data were tested by t test.For example,after treatment,the symptom improvement of the two groups was compared,and P<0.05 was considered statistically significant.Results:The effective rate of the first reduction and the total effective rate of the two groups were compared,respectively 80%and 93.33%in the manual reduction group and 93.33%and 96.67%in the machine reduction group.The P value of the two groups was greater than 0.05,and the difference was not statistically significant.The therapeutic effects of the two reduction methods were compared.In terms of patients’subjective feelings,the subjective feelings of the patients in the manipulative reduction group were 24.13±12.96 points,41.80±13.36 points,47.87±12.56 points,and53.53±13.99 points,respectively.The subjective feelings of patients in the machine reset group were 32.47±12.14 points,48.60±14.56 points,54.13±17.09 points,and 56.33±19.04 points,respectively.The P values of the two groups were 0.013,0.064,0.111 and 0.519,respectively.There was a statistical difference in the improvement of subjective feelings of the two groups on the first day after treatment,but no statistical difference in the improvement of subjective feelings on 1 week,1 month and 3 months after treatment.In terms of objective evaluation,the P values of the observed and not observed characteristic nystagmus were all greater than 0.05 in the data of patients in the two groups at 1 day,1 week,1 month and 3 months after treatment,and there was no statistically significant difference in the objective evaluation of all the statistical time points between the two groups.Conclusion:Both of the two methods can effectively treat BPPV,and there is no significant difference in the effective rate of the two methods.The improvement degree of subjective feelings of the machine reduction group on the first day after treatment was better than that of the manipulation reduction group,and there was no significant difference between the two reduction methods on the first week,the first month and the third month after treatment.There was no significant difference in the objective evaluation of improvement degree between the two groups at 1 day,1 week,1 month and 3 months after treatment. |
