Construction Of IVF-ET TCM System And Clinical Application Research Based On Six Classics Of Typhoid And Miscellaneous Diseases

Objective:1.Theoretical part:Inherit and carry forward the six dialectical academic thoughts of Treatise on Febrile and Miscellaneous Diseases,and apply it to the clinical work of traditional Chinese medicine and gynecology to explain the physiological and pathological conditions of women.Systematic application of assisted reproductive techniques(ART)provides new ideas and methods2.Clinical part:Taking traditional Chinese medicine treatment of patients with polycystic Ovary Syndrome(PCOS)with in vitro fertilization and embryo transfer(IVF-ET)as an example,it is verified the feasibility and effectiveness of the combination of the Six Classics Syndrome and IVF-ET technologies.Method:1.Theoretical part:Read classic Chinese medical literature such as Treatise on Febrile Diseases,Internal Medicine of the Yellow Emperor,Shennong’ s Materia Medica,and other philosophical books such as Yi Jing,Daode Jing,and explore the philosophy of dialectics Meaning,highlighting the significance of "time" in it,combining the Six Classics Syndrome with women’s physiological and pathological characteristics and IVF-ET technology.2.Clinical part:Design a prospective,randomized,controlled trial,analysis of clinical data of 99 patients with wet phlegm type PCOS who underwent IVF-ET due to infertility,divide them randomly into 3 groups,controlled ovarian hyperstimulation,the experimental group(group A)used the sequential treatment of traditional Chinese medicine under the guidance of Sun’s System for intervention,the control group(group B)used Cangfudaotan Decoction to intervene,the blank control group(group C)did not use Chinese medicine intervention,after 1 year of follow-up,add up and compare clinical outcomes between groups(Days of Gn medication,Gn dosage,number of eggs obtained,number of fertilizations,number of embryos,average amount of Gn required per egg,average number of Gn required per embryo,cumulative pregnancy rate,early abortion,constitution score before and after treatment),finally to make discuss and evaluate.Results:1.Theoretical part:A new theoretical system of traditional Chinese medicine(referred to as the "Sun’s System")combining the Six Classics Syndrome differentiation with the clinical practice of traditional Chinese medicine and gynecology was formed.3.Clinical part:Days of Gn administration in the three groups:group A was significantly shorter than group C(P<0.01),group B was shorter than group C(P<0.05);there was no statistical difference between groups A and B(P>0.05).The dose of Gn in the three groups:group A was significantly smaller than group B(P<0.01);group A was smaller than group B(P<0.05);there was no statistical difference between groups B and C(P>0.05).Number of eggs obtained:There was no statistical difference between groups A and B(P>0.05);both groups A and B were significantly more than those in group C(P<0.01).Number of fertilization:There was no statistical difference between groups A and B(P>0.05);there were more in groups A and B than in group C(P<0.05).Number of superior embryos:group A was significantly more than groups B and C(P<0.01);group B was more than group C(P<0.05).The average amount of Gn required per egg:There was no statistical difference between groups A and B(P>0.05),and the groups A and B were significantly less than those of group C(P<0.01).The average amount of Gn required per embryo:Group A was significantly less than Groups B and C(P<0.01);Group B was significantly less than Group C(P<0.01).There was no statistical difference in the three groups of cumulative pregnancy rate and early abortion rate(P>0.05).For patients with wet phlegm constitution,both A and B groups showed significant improvement(P<0.01),but there was no improvement in C group(P>0.05).For comparison between groups after treatment,the effect of group A was significantly better In group B(P<0.01),the efficacy of group B was significantly better than that of group C(P<0.01).Conclusion:1.Theoretical part:"Sun’ s system" can use the logical tools such as yin and yang and five elements of traditional Chinese medicine to explain women’ s physiology and pathology from the perspective of six classics syndromes.It also guides classic prescription’s systemic application in ovulation promotion and IVF-ET.Clinical part:The sequential treatment under the guidance of the "Sun’s system" can reduce the number of days of Gn medication,the amount of Gn medication,the average amount of Gn required per egg,and the average amount of Gn required per superior embryo in phlegm-wet PCOS patients undergoing IVF-ET The number of eggs obtained,the number of fertilizations,and the number of embryos improved,and the patient’s phlegm-wet constitution was improved,but the cumulative pregnancy rate and early abortion rate were not affected.Proved the feasibility,practicability,and scientificity of the"Sun’ s system".
