Objective The density of macula and perioptic capillary in normal subjects and primary open-angle glaucoma(POAG) was measured by optical optical coherence tomography angiography(OCTA) to investigate the relationship between the density of macula and peripapillary capillary as well as the damage of retinal structure and visual field in patients with open-angle glaucoma.Methods Forty-one primary open angle glaucoma(POAG) patients(77 eyes) with stable intraocular pressure from April 2018 to April 2019 in Shenzhen Aier Eye Hospital Group were selected as the observation group,while twenty-three normal subjects(43 eyes) matched by age and sex were selected as the normal control group.According to mean defect(MD),POAG patients were divided into three groups: early glaucoma group(35 eyes),advanced glaucoma group(25 eyes) and late glaucoma group(17 eyes).The mean thickness of retinal nerve fiber layer(RNFL) was measured by optical coherence tomography(OCT),and the mean density of macula and perioptic capillary was measured by OCTA.The correlation between each parameter and MD value of Humphrey 24-II standard automatic visual field measurement was analyzed.Results The mean intraocular pressure(Iop) was 14.37±2.88 mm Hg in early Glaucoma(35 eyes),the mean RNFL thickness near the optic disc was 82.97 ± 9.21 m,the mean GCC thickness in Macula was 78.69±10.16 um,and the mean visual field defect was-3.07±1.57 dB in 41 eyes(77 eyes).The mean capillary density was 46.75±3.63%,the capillary density was 49.06±4.78%,the capillary density was 48.07±5.99%,the capillary density was 46.23±33.78%,the capillary density was 52.77±7.44%,and the capillary density was 50.21±6.36% The mean capillary density in the Macula was 44.70±4.66%,the capillary density was 45.11±4.47%,the capillary density was 43.94±5.04%,the capillary density was 49.25±5.04%,and the capillary density was 42.85±4.87%.The mean intraocular pressure(Iop)was 14.00±2.33 mmhg in 25 eyes with advanced Glaucoma,the mean RNFL thickness at the side of the optic disc was 67.76±11.14 m,the mean GCC thickness at the Macula was 68.04±10.57 m,the mean visual field defect was-8.83 ±1.79 DB The mean capillary density of the optic disc was 40.37±5.97%,the capillary density was 40.56±7.62%,the capillary density was 38.55±7.07%,the capillary density was 40.13±8.63%,the capillary density was 41.87±11.50% and the capillary density was 49.80±8.63% The mean capillary density in the Macula was 37.74±4.45%,the capillary density above the macula was 38.78±4.58%,the capillary density below the Macula was 36.65±5.11%,the capillary density in the Macula was 41.92±6.91%,and the capillary density in the Macula was 35.12±4.75% The mean intraocular pressure(iop)was 14.89 ±2.87 mmhg,the mean RNFL thickness was 60.12 ±16.38 MMHG,the mean GCC thickness in Macula was 55.82 ±12.50 mmhg,the mean visual field defect was-19.71 ±5.05 DB The mean capillary density of the optic disc was 33.01±4.92%,the capillary density was 35.03±7.72%,the capillary density was 33.14±5.98%,the capillary density was 36.08±7.35%,the capillary density was 43.75±8.80%,and the capillary density was 41.12±7.89% The mean capillary density in the Macula was 33.23±3.28%,the capillary density above the macula 33.69±3.31%,the capillary density below the macula 32.95±3.68%,the capillary density in the macula 40.96±7.92%,and the capillary density in the macula 31.60±4.37% The mean iop of normal subjects(43 eyes)was 15.90±2.96 MMHG The mean capillary density of the optic disc was 52.90±1.13%,the capillary density was 51.25±2.97%,the capillary density was 51.21±2.95%,the capillary density was 47.32±3.15%,the capillary density was 56.04±2.63% and the capillary density was 53.96±3%.The mean capillary density in the Macula was 48.55±1.34%,the capillary density above the macula was 48.21±2.50%,the capillary density below the Macula was 47.37±2.32%,the capillary density in the Macula was 50.95±3.81% and the capillary density in the Macula was 47.36±3.37%.The density of the optic capillary around the optic disc in the normal control group was higher than that in the Glaucoma Group at all stages The difference was statistically significant(p < 0.01),but there was no significant difference in the vessel density of the optic disc in the normal control group(p > 0.05).Compared with the optic disc capillary density of the early Glaucoma Group,the optic disc capillary density of the Advanced Glaucoma Group and the late Glaucoma Group were significantly lower The differences were statistically significant(p 0.01).There was no significant difference in the capillary density between the early Glaucoma Group and the Progressive Group,and there was no significant difference in the capillary density between the Progressive Group and the Normal Group The density of Macula(superior,inferior,nasal and temporal)in normal control group was significantly higher than that in each stage of Glaucoma(p < 0.01)The macular capillary density in normal control group was significantly higher than that in early Glaucoma Group(p < 0.01).The macular vessel density of the normal control group was significantly higher than that of the Glaucoma Group(p < 0.01)The density of Macula(superior,inferior,nasal and temporal)in normal control group was significantly higher than that in each stage of Glaucoma(p < 0.01)There were significant differences in macular density(p < 0.01)between the sub-groups of each stage of glaucoma in comparison of the capillary(above and below).However,there was no significant difference in the density of macular capillary in temporal and nasal areas between the normal control group and the early Glaucoma Group.There was no significant difference in the density of macular capillary between the Advanced Glaucoma Group and the Advanced Glaucoma Group(p > 0.05).The mean capillary density around the optic disc was positively correlated with the mean RNFL thickness and visual field MD of the optic disc(r > 0.7,p < 0.05)In Primary Open Angle Glaucoma,the mean macular capillary density was positively correlated with the mean GCC thickness and visual field MD(r > 0.7,p < 0.05).There was a positive correlation between RNFL thickness and the density of optic disc(whole week,superior and inferior)in both early and Advanced Glaucoma Groups(r > 0.4,p < 0.05)There was a positive correlation between the mean RNFL thickness and the mean capillary density(r > 0.5,p < 0.05),and the mean RNFL thickness(r > 0.5,p < 0.05)The mean density of optic capillary was positively correlated with the MD value of visual field(r > 0.5,p < 0.05)There was a positive correlation between the macular capillary density and the mean GCC thickness(r > 0.5,p < 0.05),and between the capillary density and the Visual Field MD value(r > 0.3,p < 0.05)There was no significant correlation between macular capillary density and visual field MD in advanced and advanced Glaucoma(p > 0.05).The area under the ROC curve of optic disc capillary density and macular capillary density was greater than 0.7.Conclusion The mean capillary density of the macular region and the optic disc measured by Optical coherence Tomography angiography was significantly thinner than that of the normal population,and the mean capillary density of the macular region and the optic disc became thinner as the disease progressed There was a good correlation between RNFL and visual field damage.
