Music Therapy On Stable Chronic Obstructive Pulmonary Disease(COPD):A Meta-analysis

Background:Chronic obstructive pulmonary disease(COPD)is a common and frequently-occurring disease in community health services.It is characterized by long duration and recurrence.Rehabilitation is an important measure in treating stable chronic obstructive pulmonary disease,but patients were always having poor compliance due to heavy financial burden.Therefore,it is necessary to explore some simple and effective methods to improve the compliance as well as physical and mental health of COPD patients.Music therapy,as a non-drug therapy,is widely used in various diseases.The purpose of this article is to investigate the effects of passive music therapy(listening to music)and mixed music(combination of listening and singing)in subjects with stable chronic obstructive pulmonary disease(COPD)Methods:A systematic search in PubMed,Cochrane,Embase,Web of Science,China National Knowledge Infrastructure(CNKI),VIP,and Wanfang Data was performed in order to collect all randomized controlled trials(RCTs)about passive music therapy(listening to music)and mixed music(combination of listening and singing)in adults with COPD up to 13th February 2020.Studies selection,data extraction and risk of bias assessment were independently performed by two researchers.The risk of bias for RCTs was assessed by using Cochrane risk of bias tool,and meta-analysis was performed by using RevMan5.3.Dyspnea,exercise capacity,anxiety,depression,sleep quality and quality of life,and physiological parameters(Blood pressure态Breathing frequency and Pulmonary function)were included as outcomesResults:After initial screening of 266 potentially relevant records,13 studies with 912 participates met the inclusion criteria and were included in the final meta-analysis.Analysis showed that music therapy had a significant effect in reliving dyspnea(MD:-0.69,95%CI[-0.80,-0.58],P<0.00001),improving exercise capacity(SMD:1.49,95%CI[0.81,2.18],P<0.0001)and anxiety(SMD:-1.87,95%CI[-2.72,-1.02],P<0.0001)in subjects with COPD.Compared with the control group,music had no statistically significant in changing depression or SGRQ,while music therapy had a significant improvement when it comes to effect on improving overall quality of life of COPD patients.Besides,when it comes to effect on improving sleep quality sleep quality,the music group had better effect in reducing the total PSQI scores(MD:-3.17,95%CI[-4.14,-2.20],P<0.00001).In addition,the pooled results showed that music therapy could reduce the breathing frequency of subjects(MD:-1.98,95%CI[-3.42,-0.55],P=0.007),and had significant improvement in both systolic(MD:-7.45,95%CI[-10.95,-3.96],P<0.0001),and diastolic pressure(MD:-4.07,95%CI[-7.03,-1.12],P=0.007)between two groupsConclusions:Music therapy is efficient in reducing dyspnea,improving exercise capacity and relieving anxiety in COPD subjects.Meanwhile,music therapy can significantly improve the overall quality of life of patients.Besides,music therapy may also improve sleep quality and physiological parameters of subjects with COPD,while its role in pulmonary function still needs to be further explored.Our conclusions need to be proved further in larger and longer well-designed trials.
