ObjectiveTo develop a self-management questionnaire for patients with peptic ulcer, and to test it’s reliability and validity. The aim is to provide practical, scientific and effective tools for assessing the self-management level of patients with peptic ulcer.Methods1.This study was based on literature review and theoretical study, with the social cognition theory and self-management theory as the theoretical basis,in the framework of self-management of the three major tasks. It also referred to the evidence-based clinical practice guidelines for peptic ulcer disease(2015),consensus on the diagnosis and treatment of combined ulcers with traditional chinese and western medicine(2017)and related domestic“ self-management scales ” and “self-management questionnaires”had prepared preliminary entry pools for patients with peptic ulcers.2.The qualitative approach was used to conduct semi-structured interviews with 15 patients to fully understand their feelings and experiences on self-management of peptic ulcer disease.3.Using Delphi expert consultation method,35 experts familiar with the related fields of peptic ulcer disease were selected. After two rounds of expert questionnaire consultation, the items of the initial questionnaire were further optimized and screened.4.Using pre-survey method, twenty patients with peptic ulcer who met the inclusion and exclusion criteria were selected, and their feelings when filling in the questionnaire were pre-investigated and interviewed, and a formation of pre-test questionnaire was formed based on the results of the pre-survey and interview.5.A formal large-sample survey, through the convenience sampling method, applied to predict the questionnaire for a two level of first-class hospital endoscopic diagnosis and treatment center in shandong province into the standard of 240 patients with peptic ulcer were investigated, and Excel spreadsheets is adopted to establish the database, and using SPSS 21.0statistical software to analyze the collected data, including item analysis and screening, questionnaire reliability and validity test.Results1.Preliminary questionnaire compilation results: By referring to relevant guidelines, both at home and abroad related self-management scale and the questionnaire, literature review and the interview results of 15 patients with peptic ulcer,an initial self-management questionnaire for peptic ulcer patients with 51 items in 4 dimensions was initially developed.2.Delphi expert consultation and pre-survey results: Through 2 rounds of expert correspondence consultation and pre-survey,5 items were deleted,4items were merged, and 2 items were added, the self-management predictive questionnaire of peptic ulcer patients with 44 items in 4 dimensions has been formed.3.Item analysis and screening results: After the item screening and three exploratory factor analysis of the 44 items prediction questionnaire, deleted three entries.Eventually,a formal questionnaire for self-management of patients with peptic ulcers was formed, containing 4 dimensions,41 items.Namely, daily life management(14 items),treatment management(13items),emotion management(5 items),the disease knowledge management(9 items).4.Reliability test results: The Cronbach’s coefficient of internal consistency of the total questionnaire was 0.936,and the Cronbach’s coefficient of each dimensions was 0.854,0.956,0.888 and 0.955,indicating that the questionnaire had good reliability. The half-fold reliability coefficient of the total questionnaire was 0.941.After the questionnaire was divided into two parts according to odd even numbers, the half-fold reliability of the first part and the second part wre 0.871 and 0.877,indicating that the half-fold reliability of the questionnaire was good. The retest coefficient of the total questionnaire was 0.846,and the retest coefficient of each dimension was0.860,0.817,0.763 and 0.799,indicating that the retest reliability of the questionnaire was good and had statistical significance(P<0.05).5.Validity of the questionnaire shows as follows: Expert content validity:The content validity index(S-CVI)of the overall questionnaire level was0.93,and the content validity index(I-CVI)of each item level was0.83~1.00.Structural validity: after two exploratory factor analyses,a total of 4 common factors were extracted, the cumulative variance contribution rate was 68.187%,and the Pearson correlation coefficient between the 4 factors was 0.115~0.337,and the correlation coefficient between each factor and the total questionnaire was 0.528~0.733,with statistically significant differences(P < 0.05).Criterion validity: The correlation coefficient between the total score of the calibration scale and the total score of the questionnaire and each dimension was 0.290~0.787,and the difference was statistically significant(P<0.05).ConclusionThrough the reliability and validity test show that the peptic ulcer patients self-management questionnaire had a good result, and it can be used as a scientific and effective tool to evaluate the self-management behavior of patients with peptic ulcer, and provide a reference for further research on self-management. |
