Ancient Literature Review On Tuina Treatment Of Paediatric Fever

Objective:Various ideas in diagnosis of paediatric fever has risen up a problem of various Tuina treatment plans even in a same disease,varying among clinicians.In view of this phenomena,this study is carried out to learn the clinical experience of ancestors in treating paediatric fever using Tuina method,by carding,concluding and analysing the data collected from ancient literature review.Besides enriching the research work of ancient literatures in the field of paediatric Tuina,this study is also aimed to provide a strong reference for current clinicians.Methodology:Data are collected from ancient literatures,focusing on fundamental theories,diagnostic methods and most important Tuina treatment of paediatric fever.Carding,classification and statistical research,regarding the frequency of relevant acupoints and manipulations viewed in literature,will then be carried out.Last but not least,analysis and overall summary are crucial in order to look for a regular pattern in diagnosis and treatment of paediatric fever with Tuina among ancestors.Results:(1)Fundamental theories:Theories regarding paediatric fever are mainly Yin-Yang,five Zang and six Fu-organs,Shang-han disease and Wen-bing disease.From the aspect of Tuina treatment,theory of meridians and collaterals are closely related.(2)Diagnostic methods:Among four diagnostic methods,inspection plays the main role in diagnosing paediatric diseases,especially inspecting mental state or vitality,and complexion.In paediatric fever,red complexion with irritated expression are seen upon inspection.High-pitched sound is heard,while rapid pulse is found upon palpation.(3)Treatment method treating paediatric fever in Tuina can be divided into perspiration,heat-clearing,purgation,invigoration and harmonization methods.Among all,heat-clearing method is the most frequent used method.In perspiration method,NeiLaoGong,ErShanMen and SanGuan are frequently used.TianHeShui,ShuiDiLaoMingYue and LiuFu are among the highest frequency acupoints used to clear heat.LiuFu and large intestine acupoints are used to purge fire,while kidney and bladder acupoints are to promote urination results in clearing heat.Main acupoints used in invigoration method is FengHuangDanZhanChi,while in harmonization method,WaiLaoGong and hand YinYang are the representative acupoints.Manipulation which mostly used on point-form acupoints is fingernail-pressing method,followed by kneading method,while pushing method and circular-rubbing method are the main manipulation methods used on planar-form and line-form acupoints.(4)Keypoints of treatment are as listed.Perspiration is done by promoting,rising and dispersing Qi,resolving both exterior and interior heat.Heat-clearing method is the main method in treating fever,applicable in clearing all type of heat,regardless exterior and interior heat,excess and deficient heat.Purgation method is only applicable when there is food retention and/or with irregular bowel movements.Invigoration method can be used with heat-clearing method in treating deficient-heat.Harmonization is the core idea in treating every diseases,including fever.Ancestors enthrone paediatric Tuina but also encourage multiple choices of treatment,not being bias on any option.Conclusion:All five types of treatment method are people oriented.The primary principle is to balance the Zang-Fu organs and to harmonize YinYang,Qi and blood,achieving the purpose to eliminate pathogenic factors,treating symptoms and root of diseases.While strengthening the body,manipulation methods provide a route for pathogenic factors to be expelled from the body.This study has finally come up with a clearer outline by combining the theories and Tuina treatment of paediatric fever.This in turn helping current clinicians in better understanding ancient cultures and thoughts,so as to practice and to further develop this field.
