ObjectiveTo observe the effects of Lichang Decoction on the clinical efficacy,colonoscopy performance,intestinal flora and Th17 and Treg cell-related cytokines of patients with ulcerative colitis of spleen deficiency and dampness.To explore the mechanism of lichang decoction in the treatment of ulcerative colitis.MethodsA total of 60 patients with mild to moderate spleen deficiency and dampness-type ulcerative colitis in our outpatient and inpatient departments were collected,and the patients were divided into a treatment group and a control group of 30 cases by random number grouping.The control group received oral sulfasalazine enteric-coated tablets.The treatment group received Lichang Decoction decoction based on the treatment of the control group for a total of 8 weeks.Observe the changes in clinical efficacy,colonoscopy performance,intestinal flora and Th17 and Treg cell-related cytokines before and after treatment in the two groups.ResultsDuring the study period,2 cases were shed in the treatment group,and finally 58 cases with mild and moderate spleen deficiency and dampness syndrome of UC were clinically studied.Before treatment,the basic data(age,gender)and various observation indexes of the two groups were compared,and there was no statistical difference(P>0.05).After treatment,compared with before treatment,the disease activity index(DAI)and TCM symptom score of the two groups decreased,with significant difference(P<0.01),and the treatment group was better than the control group,with significant difference(P<0.01),while the DAI difference of the treatment group was greater than the control group,with significant difference(P<0.01).The effect of symptom improvement in the treatment group was better than that in the control group,with a significant difference(P<0.01).The total effective rate of clinical efficacy in the treatment group was 100%higher than that in the control group(80%),with a significant difference(P<0.05).After treatment,the grade of Roth in the treatment group was better than that in the control group,with significant difference(P<0.01).The numbers of IL-17,IL-21 and TGF-βin both groups decreased compared with those before treatment,while IL-10 increased compared with those before treatment(P<0.01).The numbers of IL-17,IL-21 and IL-10 in the treatment group was better than that in the control group,with significant difference(P<0.05).The decrease of TGF-β in the treatment group was better than that in the control group,but there was no significant difference(P>0.05).After treatment,the numbers of Bifidobacteria and Lactobacillus in the two groups increased compared with those before treatment,while the number of Escherichia coli and Fusobacterium nucleatum nucleatum decreased compared with those before treatment,showing a significant difference(P<0.01).Among them,the numerical improvement of Bifidobacterium,Lactobacillus and Fusobacterium nucleatum in the treatment group was better than that in the control group,with significant difference(P<0.05).The difference between Escherichia coli and Fusobacterium nucleatum in the treatment group before and after treatment was greater than that in the control group,with a significant difference(P<0.01).No adverse reactions were observed in the two groups during treatment.ConolusionTaking Lichang Decoction combined with salazosulfapyridine can improve the clinical symptoms of UC patients with mild to moderate spleen deficiency and dampness syndrome and reduce intestinal mucosal damage.The mechanism of action may be related to Lichang Decoction which can increase anti-inflammatory factors,reduce stimulating factors,increase intestinal probiotics and Related to reducing pathogenic bacteria.
