Study On The Correlationbetween Cystatin C,beta 2-Microglobulin And TCM Syndromes Of Patients With Arteriosclerosis Obliterans

Objective:To explore the correlation between cystatin C(Cys C),beta 2-microglobulin(β2-MG)and traditional Chinese medicine(TCM)syndromes of patients with arteriosclerosis obliterans.Methods:In the first study,one hundred and ninety-two ASO patients who met the criteria were selected as the observation group and 80 physical examinees as the control group.General data of the two groups were collected(sex,age,smoking history,diabetes history and hypertension history).In addition,the course,location and clinical stages of ASO patients were counted.Blood samples were collected from fasting veins of all subjects.And urea nitrogen,creatinine,uric acid,Cys C,β2-MG,fasting blood glucose,triglyceride,total cholesterol,high density lipoprotein,low density lipoprotein and homocysteine were measured.The differences of the above indexes between the two groups were compared,and the correlation between Cys C,β2-MG and ASO was further explored.The patients with ASO were stratified according to clinical stage,risk classification and different risk factors.In the second study,a total of 100 ASO patients were selected,including 50 cases of blood stasis type and 50 cases of damp-heat injection type.Then detect and observe the characteristics of the above indicators.Results:Study one shows:1.Compared with the control group,ASO patients had a higher proportion of males,smoking history,diabetes history and hypertension history(P<0.01),higher levels of age,systolic blood pressure,blood urea nitrogen,serum creatinine,Cystatin C,p2-microglobulin,fasting blood sugar and homocysteine(P<0.01),lower levels of diastolic blood pressure,glomerular filtration rate,total cholesterol,high density lipoprotein and low-density lipoprotein(P<0.01,P<0.05).2.Cys C and β2-MG levels were increased in different stages and risk classifications of ASO(P<0.01).3.The levels of Cys C and β2-MG were significantly different among different risk factors.With the increase of age,the levels of Cys C and β2-MG in ASO patients increased accordingly(P<0.01).4.Logistic regression analysis showed that sex,age,smoking,diabetes,hypertension and Cys C were independent risk factors for ASO.5.The area under the ROC curve of Cys C and β2-MG was 0.837 and 0.816.The Yoden index of Cys C is 0.567,the corresponding sensitivity is 60.4%,and the specificity is 96.3%.The Yoden index of β2-MG was 0.473,the corresponding sensitivity was 63.5%,and the specificity was 83.8%.Study two shows:Compared with blood stasis type ASO patients,damp-heat injection type patients had longer course of disease(P<0.05),higher levels of blood urea nitrogen,Cystatin C and β2-microglobulin(P<0.01),lower levels of triglyceride,high density lipoprotein and ankle-brachial index(P<0.05,P<0.01).Logistic regression analysis was used to further screen out the indicators that contributed more to TCM syndromes:Cys C,TG,HDL,ABI.Conclusion:Cys C and β2-MG are closely related to the occurrence and development of ASO.They have certain reference value for the diagnosis of ASO.Cys C and β2-MG are expressed differently in different syndromes of ASO,which can be used as an effective complement to TCM syndrome differentiation of ASO.
