Clinical Observation On Treatment Of Type 2 Diabetes Mellitus With Insulin Resistance With Huanglian Wendan Decoction And Lifting Powder

Objective: To explore the clinical effect of huanglian wendan decoction and heshengsheng powder on type 2 diabetes mellitus patients with insulin resistance(IR),and to provide reference for the diagnosis and treatment of diseases with integrated traditional Chinese and western medicine in the future.Methods: 60 patients who met the inclusion criteria were randomly divided into the control group and the experimental group with 30 patients in each group.All patients were given strict diet control combined with exercise.The control group(western medicine group)was given metformin hydrochloride tablets for oral treatment with a starting dose of 0.5g bid and a dosage of 0.5g tid which could be adjusted according to blood glucose.On the basis of taking metformin hydrochloride tablets orally in the experimental group(Chinese medicine combined with western medicine group)and the control group,the experimental group was treated with Chinese medicine(huanglian wendan decoction and heshengsheng powder plus or minus),with one dose per day and two warm doses in the morning and evening.The course of treatment in both groups was 12 weeks.The changes of fasting insulin,blood glucose,blood lipid,glycohemoglobin,body mass index,insulin resistance index and the changes of TCM syndrome score before and after treatment were observed in the two groups.Results: The comprehensive efficacy before and after treatment was compared between the experimental group(traditional Chinese medicine combined with western medicine group)and the control group(western medicine group).The total effective rate of the experimental group was 93.3%,and that of the control group was 83.3%.The comprehensive efficacy of the experimental group was significantly higher than that of the control group(P<0.01).After treatment,the total score of TCM syndromes in both groups was lower than that before treatment,and the improvement of TCM syndromes in the experimental group was significantly better than that in the control group(P=0.000<0.01).Both groups were effective in improving patients’ fasting blood glucose and blood glucose two hours after meal,and the experimental group was significantly better than the control group(P=0.000<0.01).After treatment,the cholesterol and triglycerides of the two groups were lower than before treatment,and the efficacy of the experimental group was significantly better than that of the control group(P=0.000<0.05).Insulin resistance index(ir)and body mass index(bmi)were improved before and after treatment,and the efficacy of the experimental group was significantly better than that of the control group(P=0.000<0.01).The indexes of hba1 c and fasting insulin in the two groups were improved after treatment,but there was no significant difference between the two groups.There were statistically significant differences between the two groups before and after treatment(P<0.05),but no significant differences between the two groups before and after treatment(P>0.05).At the same time,no abnormal safety indexes were observed in the two groups before and after treatment.Conclusion: Huanglian wendan decoction and shengsheng powder can effectively improve the clinical symptoms of type 2 diabetes mellitus patients with insulin resistance in clinical practice,to a certain extent,reduce glucose and fat,improve the status of insulin resistance in patients,which fully reflects the great advantages of traditional Chinese medicine in the treatment of the disease.
